摘　　要：目的探讨Periostin在非小细胞肺癌患者血清中的表达水平及其诊断价值。方法前瞻性研究。选取2018年4月至2019年11月在秦皇岛市第一医院收治的50例非小细胞肺癌患者为肺癌组。另选取同期20例肺部良性病变患者为肺部良性病变组,20名健康体检者为正常对照组。采用酶联免疫吸附法检测3组血清中Periostin水平,分析其与临床病理特征的关系,采用受试者工作特征曲线分析血清Periostin的诊断价值。结果肺癌组血清Periostin水平高于肺部良性病变组和正常对照组,差异有统计学意义(F=21.157,P<0.05);肺癌组血清Periostin水平与肿瘤大小、TNM分期、淋巴结转移相关(P值均<0.05)。血清Periostin诊断肺癌的受试者工作特征曲线下面积为0.880(95%CI:0.812~0.949,P<0.001),当截断值为27.84 mg/L时,敏感度和特异度分别为78%和85%。结论非小细胞肺癌患者血清Periostin水平显著升高,与肺癌的进展有关,可能是诊断肺癌的潜在肿瘤标志物。Objective To investigate the expression level of serum Periostin in non-small cell lung cancer(NSCLC)and daignostic value.Methods Prospective study was performed in 50 patients with non-small cell cancer(lung cancer group),20 patients with benign pulmonary disease(benign pulmonary disease group)and 20 healthy people(normal control group)who admitted to the First Hospital of Qinhuangdao from April,2018 to November,2019.The serum Periostin was determined by enzyme linked immunosorbent assay in three groups.The correlation between serum Periostin and clinicopathological factors was analyzed.The diagnostic efficacy of serum periostin was analyzed by receiver operating characteristic curve.Results The serum Periostin in lung cancer group was significantly higher than in benign pulmonary disease group and normal control groups(F=21.157,P<0.05).The level of serum Periostin statistically correlated with tumor size,TNM stage and lymphatic metastasis(all P<0.05).The area under the curve was 0.880(95%CI:0.812-0.949,P<0.001).When the cut-off value of serum Periostin level was set at 27.84 mg/L,the sensitivity and specificity were 78%and 85%,respectively.Conclusions The serum Periostin is increased in the patients with lung cancer,which is associated with the progress of lung cancer.Serum Periostin may be used as a potential tumor marker for diagnosis of lung cancer.
