摘　　要：目的探讨早期肝细胞癌(HCC)患者血清血管内皮细胞生长因子(VEGF)表达水平与预后的相关性。方法回顾性收集77例早期HCC患者临床资料,所有患者均接受实验室相关指标检测。依据患者预后情况,分为预后良好组与预后不良组,分析早期HCC患者血清VEGF水平表达与预后的相关性。结果经Logistic回归分析结果显示,血清VEGF、CEA、COX2水平升高是早期HCC患者预后不良的影响因素(OR>1,P<0.05),其中,血清VEGF水平带来的影响最显著(OR为11.689),可作为独立危险因素。绘制ROC曲线得,血清VEGF水平预测早期HCC患者预后效果的AUC为0.940,具有较高的预测价值,其特异度为0.894、灵敏度为0.910、最佳截断值为203.57μmol/l。结论血清VEGF水平与早期HCC患者预后密切相关,是早期HCC患者预后不良的影响因素,临床可加强对血清VEGF水平的监测,制定合理的预防与治疗方案。Objective To investigate the correlation between the expression level of serum vascular endothelial growth factor(VEGF)and prognosis in patients with early hepatocellular carcinoma(HCC).Methods The clinical data of 77 patients with early HCC were retrospectively collected.All patients were tested for relevant laboratory indicators.According to the prognosis of patients,they were divided into a good prognosis group and a poor prognosis group,analyzed the correlation between the expression of serum VEGF level in early HCC patients and the prognosis.Results Logistic regression analysis showed that increased serum VEGF,CEA,and COX2 levels were the influencing factors of the poor prognosis of patients with early HCC(OR>1,P<0.05).Among them,the effect of serum VEGF level was the most significant(OR=11.689),which can be used as an independent risk factor.The ROC curve was drawn,showed that the serum VEGF level predicted the AUC of prognostic effect of early HCC patients was 0.940,which with high predictive value.The specificity was 0.894,the sensitivity was 0.910,and the best cut-off value was 203.57μmol/L.Conclusion Serum VEGF level is closely related to the prognosis of early HCC patients,and it is an influencing factor for the poor prognosis of early HCC patients.The clinical monitoring of serum VEGF levels can be strengthened to provide a reliable basis for clinically formulating reasonable prevention and treatment programs.
