摘　　要：目的探讨胰腺癌患者环氧化酶-2(COX-2)和离子结合蛋白(CBL)的表达水平及预后相关性。方法选取90例胰腺癌患者为研究对象纳入胰腺癌组,以及同期胰腺炎患者50例为胰腺炎组,另选同期行健康检查的健康受试者50例为对照组。对三组患者或受试者入院后的COX-2、CBL水平进行检测对比,并对胰腺癌组患者不同分期COX-2、CBL水平进行统计对比。对胰腺癌组患者进行统一的规范化治疗,根据患者预后效果分为预后良好组和预后不良组,对两组患者的COX-2、CBL水平、无进展生存期(PFS)、总生存期(OS)进行统计对比。并对胰腺癌患者的COX-2、CBL水平与PFS、OS水平相关性进行分析。结果胰腺癌组患者的COX-2、CBL水平均高于胰腺炎组且高于对照组(P<0.05)。胰腺癌组随着肿瘤分期的增加,COX-2、CBL水平呈显著性升高的趋势(P<0.05)。预后良好组患者的COX-2、CBL水平低于预后不良组,PFS、OS水平高于预后不良组(P<0.05)。经Pearson检验分析,胰腺癌患者COX-2、CBL水平与预后指标PFS、OS呈负相关性(P<0.05)。结论COX-2、CBL可能参与胰腺癌的发生发展过程,随患者的病情恶化呈现规律性上升。COX-2、CBL与患者的预后呈现明显的相关性,可作为判断胰腺癌患者预后的参考指标。Objective To investigate the expression of cyclooxygenase-2(COX-2)and ion binding protein(CBL)in patients with pancreatic cancer and their correlation with prognosis.Methods 90 patients with pancreatic cancer were selected as research subjects and included in the pancreatic cancer group,and 50 patients with pancreatitis treated in our hospital during the same period belong to the pancreatitis group,and 50 healthy subjects who underwent health examination in our hospital during the same period were selected as the control group.The levels of COX-2 and Cbl were detected and compared among the 3 groups after admission,and the levels of COX-2 and Cbl in different stages of pancreatic cancer group were statistically compared.The patients in pancreatic cancer group were treated with unified standardized treatment.According to the prognosis,patients were divided into good prognosis group and poor prognosis group.The levels of COX-2,CBL,progression free survival(PFS)and overall survival(OS)of the two groups were statistically compared.The correlation between the levels of COX-2,CBL and PFS,OS in patients with pancreatic cancer was analyzed.Results The levels of COX-2 and Cbl in pancreatic cancer group were higher than those in pancreatitis group and control group(P<0.05).In the pancreatic cancer group,with the increase of tumor stage,the levels of COX-2 and CBL showed a significant increase trend(P<0.05).The levels of COX-2 and Cbl in the good prognosis group were lower than those in the poor prognosis group,while the PFS and OS levels were higher than those in the poor prognosis group(P<0.05).Conclusion COX-2 and Cbl may participate in the occurrence and development of pancreatic cancer,and increase regularly with the deterioration of the patient's condition.COX-2 and Cbl were significantly correlated with the prognosis of patients,which can be used as a reference index to judge the prognosis of patients with pancreatic cancer.
