摘  要:目的探讨儿童急性淋巴细胞白血病(ALL)抑癌基因ID4甲基化状态及其与疾病进展的关系。方法选择ALL患儿80例作为研究对象,根据病情程度分为慢性期32例,加速期28例,急变期20例。检测所有患儿抑癌基因-DNA结合抑制因子4(ID4)甲基化状态,随访患儿的预后并进行相关性分析。结果慢性期、加速期、急变期患儿的ID4甲基化率分别为3.1%、46.4%和90.0%,对比差异都有统计学意义(P<0.05)。80例患儿总生存时间平均为(18.29±1.22)个月,无事件生存时间平均为(10.48±1.78)个月,其中慢性期、加速期、急变期患儿的总生存时间、无事件生存时间对比差异都有统计学意义(P<0.05)。Spearman相关分析显示ID4甲基化率与临床分期、总生存时间、无事件生存时间都存在相关性(P<0.05)。多因素logistic回归方法显示ID4甲基化率、临床分期都为影响患儿总生存时间、无事件生存时间的主要影响因素(P<0.05)。结论随着病情的进展,ALL患儿的ID4甲基化率逐渐升高,且与患儿预后生存时间相关。ID4甲基化可作为ALL患儿预后评估的新的指标。Objective To investigate the methylation status of tumor suppressor gene ID4 in children with acute lymphoblastic leukemia(ALL)and its relationship with disease progression.Methods 80 children with ALL were selected as the research objects.All the cases were divided into 32 cases in the chronic phase,28 cases in the accelerated phase,and 20 cases in the blast phase accorded to the degree of disease.The methylation status of tumor suppressor gene-inhibitor DNA binding 4(ID4)were detected in all children,and the prognosis of the children were followed up and given correlation analysis.Results The ID4 methylation rates of children in the chronic phase,accelerated phase,and blast phase were 3.1%,46.4%,and 90.0%,respectively,and the differences were statistically significant(P<0.05).The average overall survival time of 80 children was(18.29±1.22)months,and the average event-free survival time was(10.48±1.78)months.Among them,the overall survival time and event-free survival of children in the chronic phase,accelerated phase,and blast phase compared the difference were statistically significant(P<0.05).Spearman correlation analysis showed that ID4 methylation rate were correlated with clinical stage,overall survival time,and event-free survival time(P<0.05).Multivariate logistic regression method showed that ID4 methylation rate and clinical stage were the main influencing factors of children's overall survival time and event-free survival time(P<0.05).Conclusion As the disease progresses,the ID4 methylation rate of children with ALL gradually increases,and is related to the prognostic survival time of the children.ID4 methylation can be used as a new indicator for the prognosis of children with ALL.
