急性非APL白血病患者抗原表达差异性分析及对疗效的影响

摘　　要：目的探讨急性早幼粒细胞性白血病(APL)以外的急性白血病(AL)患者抗原表达差异性分析及对疗效的影响。方法流式细胞术检测158例APL以外的AL患者[急性淋巴细胞白血病(ALL)28例,急性髓细胞白血病(AML)组32例]的抗原阳性表达率,分析其对疗效的影响。结果ALL中CD13、CD33阳性表达率无差异(P>0.05);AML中CD3、CD10阳性表达率高于其他抗原阳性表达率(P<0.05)。ALL化疗后有60例完全缓解;AML化疗后有67例完全缓解。ALL和AML患者中,3~5个疗程内缓解、部分缓解及未缓解者抗原阳性表达率与2个疗程内缓解者抗原阳性表达率比较均有显著差异(P<0.05)。结论急性非APL白血病患者抗原表达差异性对疗程缓解率预测有重要价值,可作为疗效判断的特异性分析。Objective To investigate the difference of antigen expression in patients with non-acute promyelocytic leukemia(APL)acute leukemia,and to discuss its effect on the curative effect.Methods Flow cytometry was used to detect the positive expression rate of antigens in 158 patients with non-APL acute leukemia,including 28 patients with acute lymphoblastic leukemia(ALL)and 32 patients with acute myeloid leukemia(AML),thereafter,its effect on treatment outcomes was analyzed.Results The positive expression rate of CD13 and CD33 of ALL patients showed no significant difference(P>0.05),while the positive expression rate of CD3 and CD10 of AML patients was higher than the positive expression rate of other antigens(P<0.05).After chemotherapy,there were 60 cases of complete response in ALL patients and 67 cases of complete response;The positive expression rates of antigens of ALL and AML patients with complete response,partial response and minor response in 3-5 courses of treatment showed significant difference with those of patients with minor response in 2 courses of treatment(P<0.05).Conclusion The difference of antigen expression in patients with non-APL acute leukemia is of great value in predicting the clinical response rate of treatment and can be used as a specific analysis for the judgment of curative effect.
