扶正化瘀胶囊对乙肝肝硬化患者肝纤维化、肠黏膜屏障功能及免疫功能的影响

摘　　要：目的:考察扶正化瘀胶囊对乙肝肝硬化患者肝纤维化、肠黏膜屏障功能及免疫功能的影响。方法:纳入2018年1月-2019年1月在江苏省南通市第三人民医院就诊的乙肝肝硬化患者86例,随机分为对照组和观察组,每组43例。对照组患者口服恩替卡韦片0.5 mg,qd,观察组患者在对照组基础上口服扶正化瘀胶囊5粒/次,tid。比较两组患者治疗后的肝纤维化指标、肠黏膜屏障功能以及免疫功能,并对两组患者治疗前和治疗后的HBV-DNA水平及转阴率进行对比。结果:治疗6个月后,两组患者的血清透明质酸(HA)、黏连蛋白(LN)、Ⅲ型前胶原(PC-Ⅲ)以及Ⅳ型胶原(Ⅳ-C)4项肝纤维化指标与二胺氧化酶(DAO)、D-乳酸以及内毒素3项肠黏膜屏障功能指标水平较治疗前均有所降低,且观察组低于对照组(P<0.05)。两组患者的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、IgA以及IgG 5项免疫功能指标均有所提高,且观察组的免疫功能指标改善情况均优于对照组(P<0.05);两组患者治疗后HBV-DNA水平均有所下降,且观察组的下降幅度显著大于对照组(P<0.05);观察组患者的HBV-DNA转阴率显著高于对照组(P=0.046)。结论:恩替卡韦联合扶正化瘀胶囊可有效缓解乙肝肝硬化患者的肝纤维化,改善肠黏膜屏障功能和免疫功能。Objective:To investigate the effects of Fuzheng Huayu capsules on liver fibrosis,intestinal mucosal barrier function and immune function in the patients with hepatitis B cirrhosis.Methods:86 patients with hepatitis B cirrhosis who sought medical care in the Nantong Third People’s Hospital of Jiangsu Province from January 2018 to January 2019 were included in the study and were randomly divided into the control group and the observation group,each consisting of 43 patients.The patients in the control group were given entecavir tablets orally,at a dosage of 0.5 mg,qd,while the patients in the observation group were treated with Fuzheng Huayu capsules,at a dosage of 5 capsules per time,tid.The indexes of liver fibrosis,intestinal mucosal barrier function and the immune function were compared between the two groups after treatment.The HBV-DNA level and negative conversion rate were compared between the two groups before and after treatment.Results:After 6 months of treatment,serum hyaluronic acid(HA),laminin(LN),procollagen typeⅢ(PC-Ⅲ)and typeⅥcollagen(Ⅵ-C),and the indexes of diamine oxidase(DAO),D-lactic acid and endotoxin concerning intestinal mucosal barrier function decreased to a certain extent,with the indexes of the observation group being lower than those of the control group(P<0.05).The immune function indexes of CD3^(+),CD4^(+),CD4^(+)/CD8^(+),IgA and IgG all improved in the 2 groups,with the immune function indexes of the observation group being superior to those of the control group(P<0.05).After treatment,the HBV-DNA level all decreased to a certain extent in the 2 groups,with the decreased level of the observation group being higher than that of the control group(P<0.05).The negative conversion rate of the observation group was significantly higher than that of the control group(P=0.046).Conclusion:Entecavir combined with Fuzheng Huayu capsules could effectively alleviate liver fibrosis and improve intestinal mucosal barrier function and immune function in the patients with hepatitis B cirrhos
