摘 要:目的探讨血清唾液酸(SA)和EB病毒壳抗原IgA(VCA-IgA)抗体检测在喉部肿瘤诊断中的临床价值。方法选取2014年2月-2019年1月广元市中心医院的经临床病理检查确诊为喉部恶性肿瘤患者85例设为恶性组,将同期经临床病理检查确诊为喉部良性病变患者85例设为良性组,对所有患者均行SA和VCA-IgA抗体检测,比较两组SA、VCA-IgA抗体水平,以ROC曲线分析SA、VCA-IgA抗体检测及其两者联合对喉部肿瘤良恶性的诊断价值,比较恶性组不同TNM病理分期、不同预后结局患者SA、VCA-IgA抗体水平差异。结果恶性组SA、VCA-IgA抗体水平明显高于良性组(P<0.05);ROC曲线分析结果显示,SA、VCA-IgA抗体检测及其两者联合对喉部肿瘤良恶性均具有一定的诊断价值(P<0.05),且SA联合VCA-IgA抗体检测的AUC大于单一的SA、VCA-IgA抗体检测,差异均具有统计学意义(P<0.05);对恶性组85例患者进行病理TNM分期,其中Ⅰ期25例,Ⅱ期34例,Ⅲ期19例,Ⅳ期7例,结果显示,Ⅰ期SA、VCA-IgA水平低于Ⅱ期,Ⅱ期SA、VCA-IgA水平低于Ⅲ期(P<0.05),Ⅲ期、Ⅳ期SA、VCA-IgA水平比较,差异无统计学意义(P>0.05);病情进展组SA、VCA-IgA水平均高于无进展组(P<0.05)。结论SA和VCA-IgA抗体检测对鉴别喉部肿瘤良恶性均具有一定的诊断价值,且SA联合VCA-IgA抗体检测的诊断效能大于单一指标检测;不同病理TNM分期患者体内SA、VCA-IgA抗体水平存在差异,SA、VCA-IgA抗体水平检测对患者预后结局具有一定预测价值。Objective To explore the clinical value of serum sialic acid(SA)and EB virus capsid antigen IgA(VCAIgA)antibodies detection in the diagnosis of laryngeal tumor.Methods A total of 85patients confirmed with malignant laryngeal tumor by clinical pathology examination in the Guangyuan Central Hospital from February 2014to January 2019were enrolled as malignant group,while another 85patients confirmed with benign laryngeal lesions were enrolled as benign group.All were tested for SA and VCA-IgA antibodies.The levels of SA and VCA-IgA antibodies were compared between the two groups.The diagnostic value of SA,VCA-IgA antibody and their combination for benign and malignant laryngeal tumor was analyzed by ROC curves.The differences in levels of SA and VCA-IgA antibodies in patients with different TNM pathological staging and prognostic outcomes in malignant group were compared.Results The levels of SA and VCA-IgA antibodies in malignant group were significantly higher than those in benign group(P<0.05).The results of ROC curve analysis showed that SA,VCA-IgA antibody and their combination were of certain diagnostic value for benign and malignant laryngeal tumors(P<0.05),AUC of SA combined with VCA-IgA antibody was greater than that of them alone(P<0.05).Among the 85patients in malignant group,there were 25cases with TNM pathological staging at stageⅠ,34cases at stageⅡ,19cases at stageⅢand 7cases at stageⅣ.The results showed that the levels of SA and VCA-IgA in patients at stageⅠwere lower than those at stageⅡ,which were lower at stageⅡthan stageⅢ(P<0.05).There were no significant differences in the levels of SA and VCA-IgA between stagesⅢandⅣ(P>0.05).The levels of SA and VCA-IgA in disease progression group were higher than those in non-progressive group(P<0.05).Conclusion The detection of SA and VCA-IgA antibodies is of certain value in differential diagnosis of benign and malignant laryngeal tumors,and the diagnostic efficiency of SA combined with VCA-IgA antibodies is greater than that of single ind
