摘 要:目的探讨研究间期癫痫样放电对癫痫患儿认知功能的影响。方法选取沧州市中心医院的140例癫痫患儿及同期体检的30名健康儿童为研究对象,就诊及体检时间均在2015年2月至2019年1月期间,其中癫痫患儿为癫痫组,健康儿童为对照组,对两组儿童均进行神经电生理检测和精神心理评估,收集两组儿童姓名、性别等基础资料及神经电生理检测结果,记录两组儿童的蒙特利尔认知评估量表各项及总得分。结果癫痫患儿中棘波指数为0的最多,66例,占比47.14%,以局灶性放电为主,共116例,占比82.86%,波幅以高幅居多,占比54.29%,散发放电110例,占比78.57%,出现在左、右侧人数一样多,均为35例,以颞部最为常见,占比64.29%;两组儿童在命名、注意力、抽象、定向力维度得分差异无统计学意义(P>0.05),癫痫组儿童语言、记忆、执行/视空间及总得分与对照组差异有统计学意义(P<0.05);棘波指数与语言、注意力、记忆、命名、总分呈负相关(P<0.05),与定向力、执行/视空间得分无相关性(P>0.05),类型、波幅、出现形式、部位与认知功能无相关性(P>0.05),侧别与执行/视空间呈负相关,与抽象、语言呈正相关(P<0.05)。结论间期癫痫样放电对癫痫患儿认知功能产生重大影响,主要在语言、记忆、执行/视空间能力等方面,棘波指数越大,患儿认知功能损害程度越大。Objective To investigate the effect of interepileptic discharge on cognitive function in children with epilepsy.Methods A total of 140children with epilepsy treated in Cangzhou Central Hospital and 30healthy children who underwent physical examination in our hospital during the same period were selected as the research subjects.For the epilepsy group and healthy children as the control group,neuroelectrophysiological testing and psychopsychological assessment were performed on the two groups of children,collecting basic data such as name and gender of the two groups of children and neuroelectrophysiological testing results,recording the Montreal cognition of the two groups of children Evaluate the scale and total score.Results Among children with epilepsy,the spike wave index was 0,66cases,accounting for 47.14%,mainly focal discharge,116cases,accounting for 82.86%,the amplitude was mostly high,accounting for 54.29%,sporadic discharge 110cases,accounting for 78.57%,with the same number appearing on the left and right sides,both of which are 35cases,the most common is the temporal region,accounting for 64.29%;the difference in scores of the two groups of children in the naming,attention,abstraction,and orientation dimensions No statistical significance(P>0.05),children with epilepsy had significant differences in language,memory,execution/visual space and total score from the control group(P<0.05);spike index and language,attention,memory,naming,The total score was negatively correlated(P<0.05),and was not correlated with directional force,performance/visual space score(P>0.05),and the type,amplitude,appearance,location and cognitive function were not correlated(P>0.05).The side is negatively correlated with execution/visual space,and positively correlated with abstraction and language(P<0.05).Conclusion Intermittent epileptiform discharges have a major impact on the cognitive function of children with epilepsy,mainly in language,memory,executive/visual spatial ability,etc.,the greater the spike wave index,the grea
