腹腔镜结直肠癌根治术患者血清CYFRA21-1、CA724水平变化与复发转移的关系

摘　　要：目的探讨腹腔镜结直肠癌根治术患者血清细胞角蛋白19片段抗原(CYFRA21-1)、糖类抗原724(CA724)水平变化与复发转移的关系。方法选取2017年1月至2019年1月重庆市巴南区人民医院腹腔镜结直肠癌根治术患者120例,同期选取体检健康人员120例,所有人员均采用放射免疫法检测血清CYFRA21-1、CA724水平,分析血清CYFRA21-1、CA724水平变化与结直肠癌根治术后复发转移的关系。结果结直肠癌根治术患者血清CYFRA21-1、CA724水平明显高于健康人员,结直肠癌根治术患者术后1天血清CYFRA21-1、CA724水平明显低于术前,差异有统计学意义(P<0.05);结直肠癌根治术患者术后随访12个月显示,复发转移有30例(25.00%),复发转移者术后3、6、12个月血清CYFRA21-1、CA724水平及上升值明显高于无复发转移者,无复发转移者术后逐渐下降,复发转移者术后逐渐升高,差异有统计学意义(P<0.05)。结论血清CYFRA21-1、CA724水平变化与结直肠癌发生及其腹腔镜根治术后复发转移有关,术后监测血清CYFRA21-1、CA724水平对复发转移具有重要的评估价值。Objective To discuss the relationship between the changes of serum cytokeratin 19fragment antigen(CYFRA21-1),carbohydrate antigen 724(CA724)levels and the recurrence and metastasis in patients undergoing laparoscopic colorectal cancer.Methods 120patients undergoing laparoscopic colorectal cancer radical operation were selected from January 2017to January 2019in Banan District People’s Hospital of Chongqing Municipality,and 120 healthy persons were selected at the same time,the serum CYFRA21-1and CA724levels were detected by radioimmunoassay,the relationship between the changes of serum CYFRA21-1and CA724levels and the recurrence and metastasis of after laparoscopic colorectal cancer was analyzed.Results The serum CYFRA21-1and CA724levels in patients undergoing laparoscopic colorectal cancer radical operation were significantly higher than those in healthy persons,and the serum CYFRA21-1and CA724levels in patients undergoing laparoscopic colorectal after operation were significantly lower than those in the before operation,the difference was statistically significant(P<0.05).The 12-month followup showed that there were 30cases(25.00%)with recurrence and metastasis.The serum CYFRA21-1and CA724levels and their rising values in the patients with recurrence and metastasis were significantly higher than those in the patients without recurrence and metastasis at 3,6and 12months after operation,the serum CYFRA21-1and CA724levels were decreased gradually in the patients without recurrence and metastasis,and the he serum CYFRA21-1and CA724 levels were increased gradually in the patients with recurrence and metastasis,the difference was statistically significant(P<0.05).Conclusion The changes of serum CYFRA21-1and CA724levels are related to the occurrence of colorectal cancer and its recurrence and metastasis after laparoscopic radical operation,the monitoring of serum CYFRA21-1 and CA724levels after operation has the important evaluation value for recurrence and metastasis.
