摘 要:目的探讨白细胞介素-16(IL-16)在原发性胆汁性胆管炎(PBC)患者肝脏和血清中的表达情况。方法收集70例患者和10名健康对照者的肝活检标本、62例患者和87名健康对照的血清标本;采用免疫组织化学法检测肝内IL-16表达情况,采用酶联免疫吸附法测定血清IL-16水平,并通过与临床指标的相关性分析确定IL-16表达水平与疾病严重程度的关联性。对正态分布的计量数据采用t检验,非正态分布数据采用Wilcoxon符号秩和检验;相关性分析采用Spearman等级相关性分析。结果IL-16在PBC患者肝脏中的表达水平显著高于健康对照组(P=0.0025),且主要表达于门管区浸润的淋巴细胞。相关性分析显示,肝组织IL-16水平与肝内炎症程度呈正相关(r=0.36,P=0.002)。此外,PBC患者血清IL-16水平较健康人群显著升高[(144.9±103.3)pg/ml与(110.1±64.8)pg/ml,P=0.0005],且血清IL-16水平与胆汁淤积标志物γ-谷氨酰转移酶水平相关(r=0.31,P=0.03)。结论PBC患者肝脏和血清IL-16的表达水平显著增加,且与疾病严重程度呈正相关,提示IL-16可能作为评估疾病严重程度的生物标志物。Objective To investigate the expressional condition of interleukin-16(IL-16)in the liver and serum of patients with primary biliary cholangitis(PBC).Methods Liver biopsies samples were collected from 70 cases and 10 healthy controls,and serum samples were collected from 62 cases and 87 healthy controls.The expression of IL-16 in liver was detected by immunohistochemistry,and the serum level of IL-16 was determined by enzyme-linked immunosorbent assay.The correlation between the expression level of IL-16 and the severity of disease was determined by correlation analysis with clinical biomarker.The t-test was used for normally distributed data.Wilcoxon signed rank sum test was used for non-normally distributed data.Results The expression level of IL-16 in the liver of PBC patients was significantly higher than that in the healthy control group(P=0.0025),and it was mainly expressed in infiltrating lymphocytes in the portal area.Correlation analysis showed that the level of IL-16 in liver tissue was positively correlated with the degree of liver inflammation(r=0.36,P=0.002).In addition,the serum IL-16 level of PBC patients were significantly higher than that of healthy people(P=0.0005),and serum IL-16 level was correlated with the level of cholestasis biomarkerγ-glutamyltransferase(r=0.31,P=0.03).Conclusion The expression level of IL-16 is significantly increased in liver and serum of PBC patients,and it is positively correlated with the severity of the disease,suggesting that IL-16 may be used as a biomarker to assess the severity of the disease.
