摘 要:目的:分析多层螺旋CT鉴别诊断肺结核空洞与肺癌空洞的临床价值。方法:回顾性分析湖北省荆州市胸科医院2017年2月-2019年11月住院或门诊治疗73例肺部空洞患者的CT影像学资料,根据空洞类型分为肺结核空洞组(38例)和肺癌空洞(35例)。所有患者均采用多层螺旋CT肺部扫描检查,比较两组肺空洞部位、壁厚、CT影像学特征及病灶临近组织情况。结果:两组肺空洞部位、壁厚等方面比较差异均有统计学意义(P<0.05);两组肺空洞在内壁光滑、外壁毛糙、钙化、结节、分叶、短毛刺、偏心性空洞及气液平面等CT影像学特征方面比较差异均有统计学意义(P<0.05);两组周围组织炎症、纵隔淋巴结肿大、纵隔淋巴结钙化、周围卫星灶及胸膜凹陷等肺空洞病灶临近组织方面比较差异均有统计学意义(P<0.05),而胸膜粘连比较差异无统计学意义(P>0.05)。结论:多层螺旋CT在鉴别诊断肺结核空洞与肺癌空洞中具有较高的临床价值。Objective:To analyze the clinical value of multislice spiral CT in the differential diagnosis of pulmonary tuberculosis cavitation and lung cancer cavitation.Method:The CT imaging data of 73 patients with pulmonary cavitation treated in Chest Hospital of Jingzhou,Hubei Province from February 2017 to November 2019 were retrospectively analyzed.According to cavitation types,they were divided into pulmonary tuberculosis cavity group(38 cases)and lung cancer cavity group(35 cases).All patients were examined by multislice spiral CT(MSCT),and the location of pulmonary cavity,wall thickness,CT imaging features and adjacent tissues of the lesions between the two groups were compared.Result:There were statistically significant differences between the two groups in lung cavity location and wall thickness(P<0.05).There were statistically significant differences between the two groups in terms of CT imaging characteristics of pulmonary cavities,such as smooth inner wall,rough outer wall,calcification,nodules,lobules,short burrs,eccentric cavitation and gas-liquid plane(P<0.05).There were statistically significant differences between the two groups in surrounding tissue inflammation,mediastinal lymph node enlargement,mediastinal lymph node calcification,surrounding satellite foci,pleural depression and other adjacent tissues of pulmonary cavity lesions(P<0.05),while there was no statistically significant difference in pleural adhesion(P>0.05).Conclusion:MSCT has a high clinical value in the differential diagnosis of pulmonary tuberculosis cavity and lung cancer cavity.
