摘　　要：目的:探讨内镜黏膜下剥离术(ESD)治疗早期胃癌患者的临床效果及对病灶切除率的影响。方法:回顾性分析本院2016年3月-2017年3月确诊并治疗的64例早期胃癌患者的临床资料,依据手术方式分为对照组(32例)和研究组(32例)。对照组采用外科切除术,研究组采用ESD术。观察比较两组术中及术后指标(手术时间、术中出血量、输血例数及住院时间)、治愈性切除率、3年生存率、并发症发生情况。结果:研究组手术时间及住院时间显著短于对照组(P<0.05);研究组术中出血量及输血占比低于对照组(P<0.05)。两组治愈性切除率、3年生存率比较差异无统计学意义(P>0.05)。研究组并发症发生率显著低于对照组(P<0.05)。结论:相比传统外科切除术,给予早期胃癌患者ESD术治疗,治愈性切除率水平及3年生存率水平相当,且并发症更少,手术时间更短。Objective:To investigate the clinical effect of endoscopic submucosal dissection(ESD)in patients with early gastric cancer and its influence on the resection rate.Method:The clinical data of 64 cases of early gastric cancer patients diagnosed and treated in our hospital from March 2016 to March 2017 were retrospectively analyzed.According to surgical method,they were divided into the control group(32 cases)and the study group(32 cases).The control group was treated with surgical resection,and the study group was treated with ESD.The intraoperative and postoperative indexes(operation time,intraoperative blood loss,number of blood transfusion and hospitalization time),curative resection rate,3-year survival rate,complications of the two groups were observed and compared.Result:The operation time and hospitalization time in the study group were significantly shorter than those in the control group(P<0.05);the intraoperative blood loss and the proportion of blood transfusion cases in the study group were significantly lower than those in the control group(P<0.05).There were no significant differences in the curative resection rate and 3-year survival rate between the two groups(P>0.05).The incidence of complications in the study group was significantly lower than that in the control group(P<0.05).Conclusion:Compared with traditional surgical resection,ESD treatment for patients with early gastric cancer has similar curative resection rate and 3-year survival rate,with less complications,shorter operative time.
