血清胃蛋白酶原、CA724、CA199联合检测在胃癌早期鉴别诊断中的价值

摘　　要：目的:探讨血清胃蛋白酶原、CA724、CA199联合检测在胃癌早期鉴别诊断中的临床意义。方法:以在笔者所在医院就诊的胃癌和一般胃部疾病患者各60例为研究对象,取静脉血检测血清中胃蛋白酶原、CA724和CA199水平,分析三种指标在胃癌早期鉴别诊断中的应用价值。结果:胃癌组的血清PGⅡ、CA724和CA199水平高于一般胃部疾病组,PGI和PGR水平低于一般胃部疾病组,组间差异均有统计学意义(P<0.05);PGR、CA724和CA199单独检测的敏感性分别为71.67%、61.67%、70.00%,均低于联合检测,组间差异均有统计学意义(P<0.05);CA724和CA199单独检测的特异性分别为68.33%、70.00%,均低于联合检测,组间差异均有统计学意义(P<0.05)。结论:胃癌患者的血清PGⅡ、CA724和CA199表达升高,PGⅠ和PGR水平降低,联合检测显示出较高的敏感性和特异性,在胃癌早期鉴别诊断中有重要临床意义和应用价值。Objective:To probe into the clinical significance of combined detection of serum pepsinogen,CA724 and CA199 in early differential diagnosis of gastric cancer.Method:A total of 60 cases of gastric cancer and 60 cases of general gastric diseases in our hospital were selected as the research objects,and the serum pepsinogen,CA724 and CA199 levels were detected in venous blood,the application value of three indexes in the early differential diagnosis of gastric cancer were analyzed.Result:The serum levels of PGⅡ,CA724 and CA199 in gastric cancer group were higher than those in general gastric disease group,and the PGI and PGR levels were lower in gastric cancer group than those in general gastric disease group(P<0.05).The sensitivity of PGR,CA724 and CA199 were 71.67%,61.67%and 70.00%respectively,which were lower than those of combined detection,and the difference between the two groups was statistically significant(P<0.05).The specificity of CA724 and CA199 were 68.33%and 70.00%respectively,which were lower than those of combined detection,and the differences between the two groups were statistically significant(P<0.05).Conclusion:The expression of PGⅡ,CA724 and CA199 in serum of patients with gastric cancer are increased,and the levels of PGⅠand PGR are decreased.The combined detection showed high sensitivity and specificity,and it has important clinical significance and application value in the early differential diagnosis of gastric cancer.
