摘  要:目的探讨尿阿尔茨海默相关神经丝蛋白(AD7c-NTP)水平和血浆同型半胱氨酸(Hcy)水平在遗忘型轻度认知损害(aMCI)诊断中的应用价值。方法选择48例aMCI为观察组,48例健康体检志愿者为对照组。检测并比较两组研究对象的尿液AD7c-NTP水平和血浆Hcy水平。采用Logistic法构建尿AD7c-NTP水平联合血浆Hcy水平诊断aMCI的联合模型,并采用受试者工作特征(ROC)曲线分析尿AD7c-NTP、血浆Hcy及联合模型诊断aMCI的效能。结果观察组的尿AD7c-NTP、血浆Hcy水平均高于对照组(均P<0.05)。采用Logistic分析法构建血浆Hcy水平联合尿AD7c-NTP水平诊断aMCI的回归模型为:Y=1/(1+e^(2.778-0.102×Hcy-1.213×AD7c-NTP))。ROC曲线分析结果显示,尿AD7c-NTP水平、血浆Hcy水平及联合模型诊断aMCI的曲线下面积分别为0.819、0.784、0.849(均P<0.05),联合模型诊断aMCI的曲线下面积均大于血浆Hcy水平、尿AD7c-NTP水平(均P<0.05)。结论aMCI患者尿AD7c-NTP水平和血浆Hcy水平均增高,两者均可作为辅助诊断aMCI的生物标志物,且两者联合可提高诊断aMCI的准确性。Objective To explore the application value of urine Alzheimer-associated neuronal thread protein(AD7c-NTP)level and plasma homocysteine(Hcy)level in the diagnosis of amnestic mild cognitive impairment(aMCI).Methods Forty-eight aMCI cases were enrolled as observation group and 48 healthy check-up volunteers as control group.The levels of urine AD7c-NTP and plasma Hcy of the subjects were detected and compared between the two groups.The Logistic method was employed to establish the combined model of urine AD7c-NTP level and plasma Hcy level for diagnosing aMCI,and the receiver operating characteristic(ROC)curve was used to analyze the efficiency of urine AD7c-NTP,plasma Hcy and the combined model for diagnosing aMCI.Results The observation group exhibited higher levels of urine AD7c-NTP and plasma Hcy than the control group(all P<0.05).Using the Logistic analysis method the regression model of plasma Hcy level combined with urine AD7c-NTP level for diagnosing aMCI was Y=1/(1+e^(2.778-0.102×Hcy-1.213×AD7c-NTP)).The results of ROC curve analysis revealed that the areas under the curve of urine AD7c-NTP level,plasma Hcy level,and the combined model were 0.819,0.784,and 0.849,respectively,for diagnosing aMCI(all P<0.05),moreover,the combined model had larger area under the curve for diagnosing aMCI than plasma Hcy level or urine AD7c-NTP level(all P<0.05).Conclusion Both urine AD7c-NTP level and plasma Hcy level are increased in aMCI patients,which can act as the biomarkers of auxiliarily diagnosing aMCI.And the combination of these two indicators can improve the accuracy of aMCI diagnosis.
