针刺法联合右美托咪定治疗结肠癌根治术后肠麻痹的效果及对血清lncRNA-MEG3表达水平的影响

摘　　要：目的观察针刺联合右美托咪定治疗结肠癌根治术后肠麻痹的疗效及对血清长链非编码RNA(lncRNA)-母系表达基因3(MEG3)表达水平的影响。方法将64例结肠癌根治术后肠麻痹患者随机分为观察组和对照组,每组32例。两组患者均采用常规治疗方法,观察组患者加用针刺联合右美托咪定治疗,均治疗1周。比较两组患者治疗效果,观察两组患者肠鸣恢复时间、术后肛门排气排便时间、治疗前后腹胀评分、腹痛评分、血清lncRNA-MEG3的表达水平。结果观察组患者治疗总有效率及术后血清lncRNA-MEG3的相对表达水平均高于对照组,且术后腹胀评分、腹痛评分、肠鸣恢复时间、术后排气和排便时间均低于或短于对照组(均P<0.05)。结论针刺联合右美托咪定治疗结肠癌根治术后肠麻痹疗效好,能有效地缓解患者腹胀和腹痛症状,促进患者胃肠功能恢复,其机制可能与上调血清lncRNA-MEG3的表达水平有关。Objective To observe the efficacy of acupuncture combined with dexmedetomidine for enteroparalysis after radical resection of colon cancer and its effect on serum long non-coding RNA(lncRNA)-maternally expressed gene 3(MEG3)expression level.Methods Sixty-four patients with enteroparalysis after radical resection of colon cancer were randomly divided into observation group and control group,with 32 cases in each group.Patients in the two groups were treated with conventional approaches,patients in the observation group received acupuncture combined with dexmedetomidine therapy additionally,and both groups were treated for one week.The curative effects were compared between the two groups,and time to bowel sound normalization,postoperative anal exhaust/defecation time,pre-and post-treatment abdominal distension score,abdominal pain score,and serum lncRNA-MEG3 expression level were observed in both groups.Results The observation group exhibited a higher total effective rate of treatment,a higher relative expression level of postoperative serum lncRNA-MEG3,lower postoperative abdominal distention score and abdominal pain score,shorter time to bowel sound normalization,shorter postoperative exhaust/defecation time as compared with the control group(all P<0.05).Conclusion Acupuncture combined with dexmedetomidine has a favorable efficacy for enteroparalysis after radical colon cancer surgery,which effectively ameliorates abdominal distention and abdominal pain symptoms and facilitates gastrointestinal function recovery in the patients.The specific mechanism may be related to the up-regulation of serum lncRNA-MEG3 expression level.
