腹腔镜下全子宫、双附件切除术治疗Ⅰ期子宫内膜癌的效果探讨

摘　　要：目的探讨腹腔镜下全子宫、双附件切除术治疗Ⅰ期子宫内膜癌的临床效果。方法78例Ⅰ期子宫内膜癌患者,按照计算机编号单双数分为观察组与对照组,各39例。对照组采用常规子宫切除术治疗,观察组采用腹腔镜下全子宫、双附件切除术治疗。比较两组术中出血量、手术时间、排气时间、术后下床时间、住院时间、并发症发生情况、生存情况及复发情况。结果观察组手术时间(62.54±5.13)min、排气时间(19.10±2.55)h、术后下床时间(2.14±0.37)d及住院时间(10.98±2.16)均短于对照组的(123.05±5.69)min、(25.43±5.07)h、(3.82±0.45)d、(16.47±2.35)d,术中出血量(132.48±26.20)ml少于对照组的(347.51±30.92)ml,差异有统计学意义(P<0.05)。观察组并发症发生率为7.69%(3/39),低于对照组的25.64%(10/39),差异有统计学意义(P<0.05)。观察组生存率92.31%、复发率5.13%与对照组的89.74%、7.69%对比,差异无统计学意义(P>0.05)。结论采用腹腔镜下全子宫、双附件切除术治疗Ⅰ期子宫内膜癌可明显缩短手术时间,减少术中出血量,安全性高,值得推广。Objective To discuss the clinical effect of total laparoscopic hysterectomy and double salpingooophorectomy in the treatment of stageⅠendometrial cancer.Methods A total of 78 patients with stageⅠendometrial cancer were divided into observation group and control group according to computer oddeven number,with 39 cases in each group.The control group was treated with conventional hysterectomy,and the observation group was treated with total laparoscopic hysterectomy and double salpingooophorectomy.The intraoperative blood loss,operation time,exhaust time,postoperative off-bed time,hospitalization time,occurrence of complications,survival and recurrence were compared between the two groups.Results The operation time(62.54±5.13)min,exhaust time(19.10±2.55)h,postoperative off-bed time(2.14±0.37)d and hospitalization time(10.98±2.16)d of the observation group were shorter than(123.05±5.69)min,(25.43±5.07)h,(3.82±0.45)d and(16.47±2.35)d of the control group,and intraoperative blood loss(132.48±26.20)ml was less than(347.51±30.92)ml of the control group.All the difference was statistically significant(P<0.05).The incidence of complications of the observation group was 7.69%(3/39),which was lower than 25.64%(10/39)of the control group,and the difference was statistically significant(P<0.05).The survival rate 92.31%and recurrence rate 5.13%of the observation group had no statistically significant difference compared with 89.74%and 7.69%of the control group(P>0.05).Conclusion For the treatment of stageⅠendometrial cancer,total laparoscopic hysterectomy and double salpingooophorectomy can significantly shorten the operation time,reduce intraoperative blood loss,and has high safety.It is worthy of promotion.
