摘　　要：目的总结血管免疫母T细胞淋巴瘤(AITL)实验室检查与临床表现的特殊性,旨在提高实验室人员和临床医生对该病的认识,减少误诊。方法回顾性分析3例AITL的临床资料、骨髓细胞学、流式细胞术结果,并复习相关文献资料,揭示AITL极易出现反应性细胞的病理生理机制。结果3例特殊的AITL病例,其骨髓以反应性的浆细胞、B细胞、T细胞增殖为主要特点,而流式检测并未检测到淋巴瘤细胞。结论AITL淋巴瘤细胞异型性大,单凭骨髓细胞学很难准确识别,对于治疗后的AITL,骨髓可能有多种反应性细胞增生,FCM才是确定细胞克隆性的重要手段。Objective To summarize the particularity of laboratory examination and clinical manifestations of angioimmunoblastic T-cell lymphoma(AITL),to reduce misdiagnosis and improve the understanding of this disease in laboratory staff and clinicians.Methods The clinical data,bone marrow morphology and flow cytometry results of three cases of AITL were retrospectively analyzed,and the relevant literature was reviewed to reveal the pathophysiological mechanism of reactive cells easily appeared in AITL.Result The bone marrow of three special AITL cases was mainly characterized by reactive plasma cells,B cells and T cells proliferation,while lymphoma cells were not detected by flow cytometry.Conclusion AITL lymphoma cells are very heteromorphic,which is difficult to be accurately identified by bone marrow morphology alone.For AITL after treatment,there may have a variety of reactive cell proliferation in bone marrow,and FCM is an important means to determine the cells clonality.
