非小细胞肺癌局部复发再程放疗的临床疗效及安全性观察

摘　　要：目的:观察首次放疗后局部复发的非小细胞肺癌再程放疗的疗效及安全性。方法:回顾性收集2004年1月~2017年6月入住我院的53例首程放疗后局部复发的非小细胞肺癌并接受再程立体定向放疗(体部伽玛刀治疗)或三维适形放疗的患者,观察再程放疗的有效性及安全性。结果:再程放疗后所有患者的1年、2年、3年的总生存率分别为81.1%、50.9%、22.6%,复发肿瘤的局部控制率分别为71.2%、30.2%、15.1%,3名患者无再次复发且无远处转移生存>5年(62、70、110个月)。再程放疗后有4名患者发生I级放射性肺炎;3名患者发生Ⅱ级放射性肺炎;没有患者发生Ⅲ级及以上放射性肺炎。结论:体部伽玛刀治疗和三维适形放疗用于再程放疗安全有效,值得临床进一步研究及推广。Objective To observe the efficacy and safety of re-irradiation for locally recurrent non-small cell lung cancer.Methods Between 2004 and 2017,53 patients with locally recurrent without distant metastasis after first radiotherapy were treated with re-irradiation(body gamma knife radiotherapy or three-dimensional conformal radiotherapy),to observe the efficacy and safety of re-irradiation.Results The overall survival rates at 1-,2-,and 3 years after re-irradiation were 81.1,50.9,22.6%,respectively,for all 53 patients.The overall local control rates at 1-,2-,and 3 years after re-irradiation were 71.2,30.2,15.1%,respectively,for all 53 patients.Three patients survived for more than 5 years(range 62-110 months)after re-irradiation,withouting further locally recurrent and distant metastasis.No patient developed grade 3 or higher toxicity after re-irradiation;radiation pneumonitis was grade 1 in 4 patients,and grade 2 in 3.Conclusions Re-irradiation appears to be a relatively safe and effective treatment,It is worthy of further clinical research and application.
