摘  要:目的:研究老年乳腺癌患者彩超血流分布特点及其与分子生物学指标的相关性。方法:选定我院在2014年7月~2019年7月收治的80例确诊为乳腺癌的老年患者作为研究对象。回顾性观察所有患者的彩超图像血流分布的特点,同时观察其分子生物学各指标[标雌激素受体(ER)、孕激素受体(PR)、肿瘤抑制基因(P53)、原癌基因(CerbB-2)、和肿瘤增殖抗原(Ki67)],分析其相关性。结果:80患者经术后病理检查确诊具有103个乳腺肿块,肿块直径0.5cm~6.5cm,其中浸润性癌73例(91.25%),包括浸润型导管癌58例,小叶癌8例,其他癌7例;原位型癌7例(8.75%),包括导管原位癌5例和小叶原位癌2例。经超声检查其中患者血流分布类型包括周边型血流18例(22.50%),穿入型血流35例(43.75%),内部型血流25例(31.25%),无血流2例(2.50%)。根据血流丰富程度其中0级2例(2.50%),Ⅰ级19例(23.75%),Ⅱ级26例(32.50%),Ⅲ级33例(41.25%)。根据具有血流信号的患者检测动脉频谱,收缩期峰值流速(PSV)为17.12±2.78cm/s,舒张末期流速(EDV)为5.76±1.67crn/s,阻力指数(RI)为0.81±0.07。80例患者中ER阳性42例(52.50%),PR阳性43例(53.75%),P53阳性45例(56.25%),CerbB-2阳性37例(46.25%),Ki67阳性66例(82.50%)。血流情况0~Ⅰ级缺乏血流组与Ⅱ~Ⅲ级血流丰富组ER、PR、P53、CerbB-2、Ki67阳性率相比具有统计学意义。经Spearman相关性分析可知,肿块血流分级与P53、Ki67阳性呈正相关关系。结论:通过对老年乳腺癌患者采用超声检测可以有利于分析和诊断乳腺肿瘤特点,分析其血流分布特点,结合其分子生物学指标相关性,有助于在一定程度上对乳腺癌的预后进行评估。Objective To study the distribution characteristics of color Doppler blood flow in elderly patients with breast cancer and its correlation with molecular biological indicators.Methods Eighty elderly patients diagnosed with breast cancer admitted to our hospital from July 2014 to July 2019 were selected as subjects.Retrospectively observing the characteristics of blood flow distribution in color Doppler images of all patients,and observing its molecular biological indicators[Estrogen receptor(ER),progesterone receptor(PR),tumor suppressor gene(P53),proto-oncogene(CerbB-2),and tumor proliferating antigen(Ki67)]were analyzed for correlation.Results 80 patients were diagnosed with 103 breast masses after pathological examination.The diameter of the tumor was 0.5cm~6.5cm,including 73 cases(91.25%)of invasive carcinoma,including 58 cases of invasive ductal carcinoma,8 cases of lobular carcinoma,and 7 cases of other cancers;there were 7 cases(8.75%)with carcinoma in situ,including 5 cases of ductal carcinoma in situ and 2 cases of lobular carcinoma in situ.Ultrasound examination showed that the patient’s blood flow distribution included peripheral blood flow in 18 cases(22.50%),penetrating blood flow in 35 cases(43.75%),internal blood flow in 25 cases(31.25%),and no blood flow in 2 cases(2.50%).According to the richness of blood flow,2 cases(2.50%)were grade 0,19 cases(23.75%)were grade Ⅰ,26 cases(32.50%)were grade Ⅱ,and 33 cases(41.25%)were grade Ⅲ.The arterial spectrum was measured according to the patient with blood flow signal.The peak systolic velocity(PSV)was 17.12±2.78 cm/s,the end-diastolic flow velocity(EDV)was 5.76±1.67crn/s,and the resistance index(RI)was 0.81±0.07.Of the 80 patients,42 were positive for ER(52.50%),43 were positive for PR(53.75%),45 were positive for P53(56.25%),37 were positive for CerbB-2(46.25%),and 66 were positive for Ki67(82.50%).The blood flow status of 0~Ⅰ-deficient blood flow group was statistically significant compared with the positive rate of ER,PR,P53,CerbB-2 and Ki6
