摘  要:为了解国内不同来源耐甲氧西林金黄色葡萄球菌(MRSA)的流行分布及耐药特征,分析其优势基因型,为有效控制MRSA流行与传播提供依据。利用常规细菌分离培养、PCR方法、微量肉汤稀释法进行MRSA的分离鉴定、药敏试验与多位点序列分型(MLST)。结果显示:(1)10省4755份样品共分离到1442株金黄色葡萄球菌,其中239株为MRSA,包括猪源MRSA 203株(18.78%),牛源27株(14.44%),鸡源9株(5.17%)。(2)多省猪源MRSA对青霉素耐药率100%,对氟喹诺酮类和头孢菌素类耐药率在80%以上,其余药物均在20%~60%;牛源/鸡源MRSA对青霉素、红霉素、克林霉素和头孢菌素类耐药率100%,但普遍对磺胺类敏感,牛源未检测到利奈唑胺耐药菌株。(3)各省份ST型分布存在差异,检测的179株MRSA共分为4个序列型,ST 9最多,在3种动物源中均有分布,其次是ST 59;ST 398和ST 1376仅出现在猪源菌株。(4)ST 9和ST 1376的亲缘关系在85%左右,同一地区、同一ST型菌株可产生不同的耐药谱。表明MRSA在我国不同地区不同动物中均有分布,猪源样品检出率最高,ST 9仍为优势型别;不同来源MRSA分离菌株存在耐药差异,且菌株基因型和耐药表型之间无相关性。To study the distribution characteristics and drug resistance of methicillin-resistant Staphylococcus aureus(MRSA)from different sources in China.Strains were isolated and identified by conventional bacterial isolation method and PCR technology.And then,micro broth dilution method was used to detect drug-resistance phenotype,and multi-locus sequence syping(MLST)technique was applied to carry out molecular type.Results showed that a total of 1442 Staphylococcus aureus(S.aureus)were isolated,239 of which were MRSA,including 203(18.78%)swine MRSA,27(14.44%)bovine MRSA,and 9(5.17%)chicken MRSA.The resistance rate of swine MRSA to penicillin was 100%,to fluoroquinolones and cephalosporins were more than 80%,and to other drugs were between 20%and 60%in many provinces.The resistance rate of bovine/chicken MRSA to penicillin,erythromycin,clindamycin and cephalosporins were 100%,which were sensitive to sulfonamides.There was no resistant bovine strain to linezolid detected.Four STs were obtained from 179 strains tested by MLST.ST 9 was the most popular and distributed in all three animal sources,then followed by ST 59.ST 398 and ST 1376 were distributed only in swine strains.The relationship between ST 9 and ST 1376 was about 85%by cluster analysis.MRSA distributed in different animals and different regions in China,and the detection rate from pigs was the highest,while ST 9 was still the dominant type.Differences in drug resistance existed between different sources,and no correlation was observed between genotype and drug resistance.
