压膜保持器与HAWLEY保持器联合舌侧保持丝的正畸治疗后保持效果比较

摘　　要：目的:对比探讨压膜保持器和HAWLEY保持器联合舌侧保持丝在正畸治疗后的保持效果。方法:选取2016年2月-2018年9月口腔正畸治疗结束的患者80例,随机分为两组,各40例。对照组采用压膜保持器;试验组采用HAWLEY保持器联合舌侧保持丝。保持1年后,比较两组Little前牙不齐指数、尖牙间宽度、磨牙间宽度及牙弓长度。结果:两组保持器戴用前和戴用1年后的前牙不齐指数比较,差异有统计学意义(P<0.05);两组保持器戴用前和戴用1年后的尖牙间宽度、磨牙间宽度及牙弓长度比较,差异无统计学意义(P>0.05)。结论:若使前牙更好地保持或后牙需要垂直方向自行调整,且保持器不易损坏,可以选择HAWLEY保持器联合舌侧保持丝进行保持。若矫治前后前牙整齐程度区别不大,可采用压膜保持器,可以考虑制作两副,以达到较长久的保持效果。临床上正畸医生可以根据患者具体情况采用合适的保持器。Objective:To compare the clinical effectiveness of vacuum-formed retainer and HAWLEY retainer combined with lingual retaining wire after orthodontic treatment.Methods:From February 2016 to September 2018,80 cases of malocclusion patients after orthodontic treatment were enrolled,they were randomly divided into two groups with 40 cases in each group.The control group used vacuum-formed retainer.The experimental group applied HAWLEY retainer combined with lingual retaining wire.After one year,the Little irregularity index,the width of the canine tooth arch,the width of the molar arch and dental arch length were compared between the two groups and statistically analyzed.Results:After one year of retention,the difference of the Little irregularity index between the two groups was compared,the difference was statistically significant(P<0.05).After one year of retention,the width of the canine tooth arch,the width of the molar arch and dental arch length between two groups were compared,the difference were not statistically significant(P>0.05).Conclusion:If the anterior teeth need to better maintained or the posterior teeth need to be adjusted vertically,and the retainer is not easily damaged,HAWLEY retainer combined with lingual retaining wire can be selected for retention.If the difference in the orderliness of the anterior teeth before and after treatment is not large,the vacuum-formed retainer can be used,and two pairs can be considered for a longer-term retention effect.Clinical orthodontists can use appropriate retainers according to the patient's specific conditions.
