摘　　要：目的应用超声联合神经刺激仪技术,观察闭孔神经阻滞(ONB)在经尿道膀胱肿瘤电切术(TURBT)中的临床效果。方法选择2018年5月—2019年5月在潍坊市人民医院拟行椎管内麻醉下TURBT的患者60例,随机分为超声引导组和超声联合神经刺激仪引导组(简称联合引导组),每组30例。两组均采用脊柱-硬膜外复合麻醉联合ONB的麻醉方式。记录神经阻滞操作时间、手术时间、一次穿刺成功率,以及阻滞成功率;观察麻醉和手术并发症;调查术者满意率。结果联合引导组的神经阻滞操作时间显著长于超声引导组(P<0.05)。联合引导组的一次穿刺成功率和阻滞成功率均显著高于超声引导组(P值均<0.05)。结论超声联合神经刺激仪实施ONB用于TURBT能够明确穿刺部位,提高阻滞成功率。Objective To observe the application of obturator nerve block(ONB)in the treatment of transurethral resection of bladder tumor(TURBT)by using ultrasound combined with neurostimulator.Methods Sixty patients who underwent TURBT under spinal anesthesia from May 2018 to May 2019 in Weifang People’s Hospital were selected and randomly divided into two groups.ONB was performed by ultrasound guidance in 30 patients(ultrasound-guided group),and by ultrasound guidance and neurostimulator in other 30 patients(combined guided group).The duration of ONB,operation time,success rate of puncture and success rate of blocking were recorded.The complications of anesthesia and operation were observed.The operators’satisfaction was investigated.Results The duration of ONB in the combined guided group was significantly longer than that in the ultrasound-guided group(P<0.05).The success rates of initial puncture and blocking in the combined guided group were significantly higher than those in the ultrasound-guided group(both P<0.05).Conclusion ONB guided by ultrasound combined with neurostimulator can identify the puncture site and improve the blocking efficiency.
