摘  要:目的探讨依达拉奉对微创颅内血肿清除术后患者神经功能及氧化应激的影响。方法纳入150例高血压脑出血(HICH)患者,观察组75例和对照组75例,对照组患者采用微创颅内血肿清除术治疗,观察组在对照组基础上静滴依达拉奉治疗。对比2组病情程度评分、血清神经功能指标、氧化应激指标及血肿、水肿情况,统计2组临床治疗效果。结果观察组术后15 d时NIHSS评分、mRS评分低于对照组(t=8.055、2.224,P<0.05),MMSE评分明显高于对照组(t=4.775,P<0.05)。观察组术后血肿体积及水肿面积均明显小于对照组(t=4.165、8.148,P<0.05)。与对照组相比,观察组术后血清脑源性神经营养因子(BDNF)、神经生长因子(NGF)水平显著升高(t=3.304、9.304,P<0.05),血清神经元特异性烯醇化酶(NSE)、丙二醛(MDA)、髓过氧化物酶(MPO)水平则明显降低(t=4.368、6.432、6.857,P<0.05)。观察组总有效率92.00%,高于对照组的81.33%(χ^(2)=5.181,P<0.05)。结论HICH患者微创颅内血肿清除术后静滴依达拉奉治疗能有效降低氧化应激水平,促进神经功能恢复,提高临床疗效。Objective To investigate the influence of edaravone on neural function and oxidative stress in patients undergoing minimally invasive intracranial hematoma removal.Methods were divided into observation group(75 cases)and control group(75 cases).The control group was treated by minimally invasive intracranial hematoma removal,while the observation group was treated with idaravone intravenous injection on the basis of control group.The disease degree score,serum neurological function indexes,oxidative stress indexes,hematoma and edema were compared between two groups.The clinical therapeutic effects of two groups were statistically analyzed.Results The NIHSS and mRS scores of observation group were lower than control group at 15 days after the operation(t=8.055,2.224,P<0.05),while the MMSE scores were significantly higher than control group(t=4.775,P<0.05).The postoperative hematoma volume and edema area in observation group were also significantly smaller than control group(t=4.165,8.148,P<0.05).Compared with control group,the serum levels of brain-derived neurotrophic factor(BDNF)and nerve growth factor(NGF)were significantly increased in observation group(t=3.304,9.304,P<0.05),while The levels of neuron-specific enolase(NSE),malondialdehyde(MDA)and myeloperoxidase(MPO)were significantly reduced(t=4.368,6.432,6.857,P<0.05).The total effective rate of observation group was 92.00%,which was higher than control group by 81.33%(χ^(2)=5.181,P<0.05).Conclusion Edaravone treatment after minimally invasive intracranial hematoma removal in HICH patients can effectively reduce the level of oxidative stress indexes,promote the recovery of nerve function and improve the clinical effect.
