摘 要:目的应用学习曲线理论探索新入职护士熟练掌握外周静脉留置针穿刺技术需达到的最少操作例数,从而为临床技能培训提供依据。方法采用计算机程序随机抽取南京医科大学第一附属医院老年心血管科2017年新入职护士10名,由2位老师对研究对象进行操作项目的培训,将此操作项目连续操作50例次,按照操作的先后顺序分为5个阶段,每个阶段10例次作为一组,依序进行编号,5个阶段对应编号分别为A组(编号1~10)、B组(编号11~20)、C组(编号21~30)、D组(编号31~40)、E组(编号41~50),比较5个阶段操作的时间、得分和成功率。结果采用SNK-q检验进行组间两两比较,发现达到30例时,操作时间、操作得分及一次性穿刺成功率均出现拐点。结论外周静脉留置针操作的学习曲线约为30例,即可达到熟练的程度。Objective Application of learning curve theory to explore the need for new nurses to master the peripheral venous indwelling needle puncture technique to achieve the minimum number of operation cases,so as to provide a basis for clinical skill training.Methods Ten newly recruited nurses from the Geriatric Cardiovascular Department of the First Affiliated Hospital with Nanjing Medical University in 2017 were randomly selected by computer program.Two teachers trained the subjects in the operation project.This operation item was operated for 50 consecutive times,which was divided into 5 stages according to the sequence of operations.Each stage of 10 cases was taken as a group and numbered according to the sequence.The five stages were numbered as group A(No.1~10),group B(No.11~20),group C(No.21~30),group D(No.31~40)and group E(No.41~50)respectively.The operation time,scores and success rate of the five stages were compared.Results SNK-q test was used for pairwise comparison between the groups,and it was found that when the number of patients reached 30,the inflection point of operation time,operation score and one-time puncture success rate all appeared.Conclusions The learning curve of peripheral venous indwelling needle operation is about 30 cases,and the proficiency level can be reached.
