磁共振成像扩散加权成像联合肿瘤标记物对结直肠癌术前肝转移的早期诊断价值

摘　　要：目的探究磁共振成像(MRI)扩散加权成像联合肿瘤标记物对结直肠癌术前肝转移的早期诊断价值。方法选取我院于2019年1月至2020年5月收治的30例结直肠癌患者作为本次的研究对象,术前检测患者肿瘤标志物癌胚抗原(CEA)、糖类抗原(CA)125、CA19-9的水平,记录并分析患者的MRI扩散加权成像特点,评价单项和两项联合诊断结直肠癌肝转移的敏感度、特异性以及准确率。结果单项检测患者肿瘤标记物后得出:CA19-9的敏感度最高,为73%,CA125的特异性最高,为93%,而CEA、CA125、CA19-9三种标记物联合检测后,敏感度可达93%,特异性可达100%;单独采用MRI扩散加权成像准确率为90%(27/30),采用CEA、CA125、CA19-9三种标记物联合检测的准确率为93%(28/30),而采用MRI扩散加权成像联合肿瘤标记物后的准确率为100%,其中P<0.05。结论采取MRI扩散加权成像联合肿瘤标记物对结直肠癌术前肝转移进行早期诊断,具有更高的敏感度、特异性以及准确率,为后续临床治疗提供有力保障。Objective To explore the value of MRI diffusion-weighted imaging combined with tumor markers in the early diagnosis of liver metastasis of colorectal cancer.Methods Thirty patients with colorectal cancer admitted in our hospital from January 2019 to May 2020 were selected as the subjects of this study.The levels of tumor markers CEA,CA125,CA19-9 in patients before surgery were detected,and the patients′MRI spread the weighted imaging characteristics were recorded and analyzed,and the sensitivity,specificity,and accuracy of the single and two joint diagnosis of colorectal cancer liver metastasis were evaluated.Results After a single test of patient tumor markersv showed,that CA19-9 had the highest sensitivity of 73%,CA125 had the highest specificity of 93%,and CEA,CA125,CA19-9 combined detection of three markers,the sensitivity can reach 93%,specificity can reach 100%;the accuracy rate of MRI diffusion weighted imaging alone was 90%(27/30),and the accuracy rate of combined detection using CEA,CA125,CA19-9 three markers was 93%(28/30)The accuracy rate after using MRI diffusion weighted imaging combined with tumor markers was 100%,P<0.05.Conclusion MRI diffusion weighted imaging combined with tumor markers for early diagnosis of colorectal cancer preoperative liver metastasis has higher sensitivity,specificity and accuracy,and provides a powerful guarantee for subsequent clinical treatment.
