自贡地区女性高危型人乳头状瘤病毒流行病学特征分析

摘　　要：目的通过分析四川省自贡市女性高危型人乳头状瘤病毒(hr-HPV)感染状况及基因分型分布特征,为本地区宫颈癌前病变和宫颈癌的筛查、预防等提供理论依据。方法选取2018年1月至2020年1月自贡地区女性健康体检人员和门诊病人共9201例(健康体检人员2694例,门诊病人6507例),实时荧光定量PCR法进行检测,并对hr-HPV感染率及基因分型进行分析。结果9201例标本中检出阳性1190例,总感染率12.93%,其中体检组202例(7.50%),门诊组988例(15.18%),门诊组感染率大于体检组(P<0.001);亚型感染率以HPV16最高,其次为HPV52、58和68;感染以单一感染为主;hr-HPV感染率各年龄段呈现“两头高中间低”趋势,在小于25岁年龄段较高(20.76%),之后随着年龄增高而降低,在>50~55岁年龄组又升高,以>60~65岁年龄组最高(24.78%)。且年龄组hr-HPV感染分布差异有统计学意义(χ^(2)=53.231,P<0.001)。结论四川省自贡市女性hr-HPV感染率相对较低,分型以HPV16、52、58、68为主,型别分布与全国其他地区、与四川省整体及四川其他地区略有不同,且要特别重视自贡地区51岁以上中老年妇女hr-HPV感染的预防。Objective this region by analyzing on the infection status and genotype distribution of human papilloma virus(hr-HPV).Methods A total of 9201 health physical examination and outpatient women(physical examination personnel 2694 cases,6507 patients with clinic)in Zigong Region,Sichuan province from January 2018 to January 2020 were analyzed.The cervical exfoliated cells of the subjects were collected.The HPV DNA was tested by real-time fluorescent quantitative PCR.The women’s hr-HPV infection rate and genotyping were analyzed.Results Among the 9201 samples,1190 samples were positive,with a total infection rate of 12.93%,including 202 in the physical examination group(accounting for 7.50%)and 988 in the outpatient group(accounting for 15.18%).The infection rate in the outpatient group was significantly higher than that in the physical examination group(χ^(2)=99.937,P<0.001).The rate of subtype infection was highest in HPV16,followed by HPV52,HPV58 and HPV68.The majority of infections were single infection.In all ages,the hr-HPV infection rate showed a trend of"high at both ends and low in the middle",which was higher in the age group less than 25 years old(20.76%),then decreased with increasing age,but increased in the age group>50-55 years old,reaching the highest in the age group>60-65 years old(24.78%),and the difference of hr-HPV in each age group was statistically significant(χ^(2)=53.231,P<0.001).Conclusion In general,the hr-HPV infection rate of women is relatively low in Zigong region.The main subtypes are HPV16,52,58,68 and genotype distribution is slightly different from that in the rest of our country,and in other parts of the whole Sichuan province.In addition,we should pay special attention to the prevention of hr-HPV infection in middle-aged and elder women over 51 years of age in Zigong region.
