双气囊小肠镜与磁共振成像对小肠出血性疾病的诊断价值

摘　　要：目的评价双气囊小肠镜与磁共振小肠成像在小肠出血性疾病临床诊断中的应用价值。方法回顾性分析首都医科大学附属北京朝阳医院2014年6月至2019年5月收治的42例确诊为小肠出血性疾病患者的临床资料,对比分析双气囊小肠镜检查与磁共振成像对小肠出血性疾病的诊断价值。结果42例小肠出血性疾病患者中,小肠肿瘤性疾病19例(45.24%),小肠血管性疾病14例(33.33%),小肠炎性疾病4例(9.52%),小肠其他疾病5例(11.90%)。双气囊小肠镜检查诊断阳性率为92.86%(39/42),磁共振成像诊断阳性率为83.33%(35/42),二者检查准确率比较差异无显著性(P>0.05)。结论双气囊小肠镜检查和磁共振成像均为诊断小肠出血性疾病的成熟技术,二者联合应用可以优势互补,提高病变的检出率。Objective To evaluate the clinical value of double-balloon enteroscopy(DBE) and magnetic resonance imaging(MRI) in the diagnosis of small intestinal hemorrhagic diseases. Method The clinical data of 42 patients with small intestinal hemorrhagic diseases admitted to Beijing Chaoyang Hospital from June 2014 to May 2019 were retrospectively analyzed. The diagnostic value of DBE and MRI in small intestinal hemorrhagic diseases was compared and analyzed. Result Among the 42 cases of small intestinal hemorrhagic diseases, 19 cases(45.24%) were small bowel tumor diseases, 14 cases(33.33%) were small intestinal vascular diseases, 4 cases(9.52%) were small intestinal inflammatory diseases, and 5 cases(11.90%) were other small intestinal diseases. The accuracy rate of DBE and MRI was 92.86%(39/42) and 83.33%(35/42), respectively, and there was no statistically significant difference between the two methods(P>0.05). Conclusion DBE and MRI are mature examination techniques for the diagnosis of small intestinal hemorrhagic diseases. The combined application of the two examination methods can complement each other and improve the detection rate.
