摘　　要：目的探讨胃癌组织中巨噬细胞加帽蛋白G(CapG)、同源异型盒基因转录因子1(Prox-1)、酪氨酸激酶受体B(TrkB)的表达对胃癌淋巴结转移(LNM)及预后的影响。方法回顾性分析2014年1月至2015年4月在锦州医科大学附属第一医院术前活检确诊并行根治术治疗的110例胃癌患者临床资料。于术前对胃癌活检组织中巨噬细胞加帽蛋白G(CapG)、酪氨酸激酶受体B(TrkB)、同源异型盒基因转录因子1(Prox-1)、基质金属蛋白酶2(MMP-2)、血管内皮生长因子C(VEGF-C)及血管内皮生长因子受体3(VEGFR-3)的表达进行检测。采用受试者工作特征曲线(ROC曲线)与Kaplan-Meier生存曲线分析不同分子生物标志物对胃癌LNM及预后的影响。结果LNM患者CapG、Prox-1、TrkB、MMP-2、VEGF-C及VEGFR-3的阳性表达率均明显高于无LNM患者(P<0.05)。CapG、Prox-1、TrkB对胃癌LNM的预测价值较高。CapG、Prox-1及TrkB的阳性表达患者总生存率均明显低于阴性表达患者(P<0.05)。CapG阳性表达(HR=6.524,95%CI:1.012~36.362,P=0.027)是胃癌患者预后的独立危险因素。结论术前检测CapG、Prox-1及TrkB表达对胃癌LNM及预后具有较高的预测价值。Objective To investigate the effect of expression of CapG,Prox-1 and TrkB on lymph node metastasis(LNM)and prognosis of gastric cancer.Methods The clinical data of 110 patients with gastric cancer confirmed by preoperative biopsy and radical resection in the hospital from January 2014 to April 201 were retrospectively analyzed.The expressions of capping protein G(CapG),tyrosine kinase receptor B(TrkB),prospero-related homeobox 1(Prox-1),matrix metalloproteinase 2(MMP-2),vascular endothelial growth factor C(VEGF-C)and vascular endothelial growth factor receptor 3(VEGFR-3)in gastric cancer biopsy tissue before surgery were detected.The ROC curve and Kaplan-Meier survival curve were used to analyze the effects of different molecular biomarkers on LNM and prognosis of gastric cancer,respectively.Results The positive expression rates of CapG,Prox-1,TrkB,MMP-2,VEGF-C and VEGFR-3 in the patients with LNM were significantly higher than those in the patients without LNM(P<0.05).CapG,Prox-1,TrkB had highly predictive value on LNM of gastric cancer.The overall survival rate of the patients with positive expression of CapG,Prox-1 and TrkB was significantly lower than that of the patients with negative expression(P<0.05).The positive expression of CapG(HR=6.524,95%CI:1.012-36.362,P=0.027)was an independent risk factor for the prognosis of the patients with gastric cancer.Conclusion Preoperative detection of CapG,PROx-1 and TrkB expression has higher predictive value for LNM and prognosis of gastric cancer.
