当归补血汤治疗骨科术后血虚发热的Meta分析

摘　　要：目的:系统评价当归补血汤治疗骨科术后血虚发热的临床疗效。方法:计算机检索包括中国期刊全文数据库(CNKI)、万方数据库(WANGFANG)、维普数据库(VIP)、中国生物医学文献数据库(CBM)、Cochrane Library、Pubmed、Embase等数据库中关于当归补血汤治疗骨科术后血虚发热的随机对照试验(Randomized controlled trial,RCT),各数据库检索时间范围均限定为建库至2020年3月,筛选出符合纳入标准的文献,对纳入文献采用Jadad评分表进行质量评价,使用Review Manager 5.3软件进行Meta分析。结果:共纳入10项RCT,801例患者,治疗组401例,对照组400例进行Meta分析。结果:与对照组相比,使用当归补血汤治疗骨科术后血虚发热能够更好地降低患者体温(95%CI为[-0.78,-0.47],P<0.05)、提升治疗总有效率(95%CI为[3.37,10.92],P<0.05)、降低血虚发热症状积分(95%CI为[-3.54,-2.67],P<0.05)、提高血红蛋白含量(95%CI为[1.21,2.05],P<0.05)、提高红细胞压积(95%CI为[0.06,0.08],P<0.05)。结论:当归补血汤在治疗骨科术后血虚发热上能够提升血红蛋白含量、红细胞压积,有效降低患者体温,提高治疗有效率。但仍需开展严谨的高质量、大样本随机对照试验,进一步完善安全性评价,从而提供更有力的证据评估当归补血汤治疗骨科术后血虚发热的临床疗效。Objective To systematically evaluate the clinical efficacy of Danggui Buxue Decoction in the treatment of blood deficiency and fever after orthopedic operation.Methods Computer search includes the randomized controlled trial(RCT)of Danggui Buxue Decoction in the treatment of blood deficiency and fever after orthopedic operation in China National Knowledge Infrastructure(CNKI),Wanfang Database(WANGFANG),VIP Database of Chinese journal(VIP),Chinese Biomedicine Database(CBM),Cochrane Library,Pubmed,Embase and other databases.The search time range of each database is limited to the period from the establishment of the database to March 2020.The documents that meet the inclusion criteria are selected.Evaluation,meta-analysis using Review Manager 5.3 software.Results A total of 10 RCTs were included,801 patients,401 patients in the treatment group,and 400 patients in the control group.A meta-analysis was performed.The results showed that compared with the control group,the use of Danggui Buxue Decoction to treat blood deficiency and fever after orthopedic surgery was better Decrease patient temperature(95%CI[-0.78,-0.47],P<0.05),increase total effective rate of treatment(95%CI[3.37,10.92],P<0.05),reduce blood deficiency and fever symptom score(95%CI is[-3.54,-2.67],P<0.05),increase hemoglobin content(95%CI is[1.21,2.05],P<0.05),increase hematocrit(95%CI is[0.06,0.08],P<0.05),the difference is statistically significant.Conclusion Danggui Buxue Decoction can increase hemoglobin content and hematocrit in the treatment of blood deficiency and fever after orthopedic surgery,effectively reduce the body temperature of patients,and improve the treatment efficiency.However,rigorous high-quality,large-sample randomized controlled trials are still needed to further improve the safety evaluation,so as to provide stronger evidence to evaluate the clinical efficacy of Danggui Buxue Decoction in treating blood deficiency and fever after orthopedic surgery.
