摘 要:目的分析UPPP+颏前徙术和UPPP+下颌升支矢状劈开前移术+颏前徙术两种术式对OSAHS伴小颌畸形患者骨组织移动距离与上气道形态变化的相关性。方法对22例OSAHS伴小颌畸形患者,分成两组,A组为15例行UPPP+颏前徙术的OSAHS患者,B组为7例UPPP+下颌升支矢状劈开前移术+颏前徙术的OSAHS患者,通过三维重建CT测量两组患者术前、术后舌后区气道矢状径距离、横截面积及矢状面积,对不同术式中OSAHS患者术前、术后骨组织向前移动距离与上气道软组织的变化进行相关性分析。结果两种术式术前、术后骨组织移动的前后距离与上气道软组织的变化具有正相关性,具有统计学意义(P<0.05),且骨组织移动距离越多,上气道软组织的变化越大。结论对于OSAHS伴小颌畸形患者,可适当增加骨组织移动距离改善上气道状况,以取得更好的疗效,为临床工作中术式及术中操作的选择提供科学的依据。Objective To analyze the correlation between translation of bone fragments and morphologic change of upper airway in OSAHS patients with micrognathia deformity who underwent UPPP+chin advancement or UPPP+sagittal split ramus osteotomy(SSRO)+chin advancement. Methods We retrospectively reviewed 22 patients with OSAHS who underwent orthognathic surgery. They were divided into 2 groups according to different methods: In Group A, 15 patients underwent UPPP +chin advancement. In Group B, 7 patients with UPPP +sagittal split ramus osteotomy(SSRO)+chin advancement. To analyze the correlation, by measuring the pre-and post-retro-glottal region sagittal distance, cross-section and sagittal area by reconstruct of CT data. Results The average translation of bone fragments was positively correlated with the morphological change of upper airway soft tissue(P<0.05). Conclusion For OSAHS patients with micrognathia deformity, it could get better results for properly increase the translation of the bone fragments to improve the upper airway condition. It also could provide scientific basis for choosing surgical methods in clinical work.
