摘 要:目的探讨颈部核心稳定训练治疗颞下颌关节紊乱病的疗效。方法选取符合TMD诊断标准的患者212例,年龄18~60周岁,采用区组随机法分为对照组和治疗组各106例。对照组给予超短波+颞下颌关节手法治疗,治疗组给予超短波+颞下颌关节手法治疗+颈部核心稳定训练。在治疗前、治疗后及治疗后3个月时评估所有患者的数字疼痛评定量表(NPRS)、最大张口度(MMO)及颞下颌功能限制量表(JFLS),在治疗后及治疗后3个月记录所有患者的总体症状改变等级评分(GRCS)。结果 2组治疗后及治疗后3个月NPRS评分比较差异有统计学意义(P<0.05)。2组治疗后及治疗后3个月JFLS评分比较差异有统计学意义(P<0.05),2组治疗后及MMO比较差异有统计学意义(P<0.05)。GRCS评分结果显示,2组治疗后及治疗后3个月比较差异有统计学意义(P<0.05)。结论以颈部核心稳定性训练为基础治疗模式能够早期缓解TMD患者的疼痛,改善关节功能及最大张口度,患者满意度高,并且能够获得较好的远期疗效。Objective To investigate the effect of neck core stabilization training on temporomandibular joint disorders. Methods Two hundred and twelve TMD patients, aged 18 ~60 years old, were randomly divided into control group(106 cases) and treatment group(106 cases). The control group was treated with ultrashort wave and temporomandibular joint manipulation, while the treatment group was treated with ultrashort wave and temporomandibular joint manipulation and neck core stability training. The Numeric Pain Rating Scale(NPRS), Maximum Mouth Opening(MMO)and Jaw Functional Limitation Scale(JFLS)were evaluated before treatment, after treatment and 3 months after treatment. The Global Rating of Change Scales(GRCS) of all patients was recorded after treatment and 3 months after treatment. Results There was significant difference in NPRs scores between the two groups after treatment and 3 months after treatment(P <0.05). There was significant difference in jfls score between the two groups after treatment and 3 months after treatment(P<0.05), and there was significant difference between the two groups after treatment and MMO(P<0.05). GRCs score showed that there was significant difference between the two groups after treatment and 3 months after treatment(P <0.05). Conclusion The neck core stability training can relieve the pain of TMD patients early,improve joint function and maximum mouth opening, and with high patient satisfaction and good long-term effect.
