摘 要:目的比较计算机辅助设计与制造(computer aided design and computer aided manufacturing,CAD/CAM)IPS e.max CAD髓腔固位的高嵌体和Lava Ultimate髓腔固位的高嵌体修复根管治疗后牙体缺损的临床效果。方法选取根管治疗后缺损磨牙90颗,随机均分为两组,分别制作CAD/CAM IPS e.max CAD和Lava Ultimate髓腔固位的高嵌体。根据改良美国公共卫生署(United States Public Health Service,USPHS)标准,比较两组材料修复后6个月、12个月的成功率。结果在不同复诊时间段,两组在边缘密合性、修复体完整性、基牙完整性、颜色匹配、继发龋、牙龈炎症和对颌牙磨耗方面,差异无统计学意义(P>0.05),其中12个月复诊时仅IPS e.max CAD组出现一例髓腔固位的高嵌体脱落,Lava Ultimate组出现一例功能尖小面积崩损。其余修复体均效果良好,无脱落折裂情况。结论 IPS e.max CAD和Lava Ultimate髓腔固位的高嵌体皆可用于修复无髓后牙缺损,短期临床效果良好。Objective To compare the clinical efficacy of IPS e.max CAD and LavaTM Ultimate composite CAD/CAM(computer aided design and computer aided manufacturing) onlay restorations with pulp chamber retention in repairing posterior pulpless teeth. Methods A total of 90 defective molars after root canal therapy were randomly divided to two groups, which were respectively restored by CAD/CAM IPS e.max CAD(n=45) and LavaTMUltimate onlays(n=45) with pump chamber retention. The successful rate of onlay restoration at 6 months and 12 months was calculated based on the U.S. Public Health Service(USPHS) standards. Results There were no significant differences in the marginal adaptation, structural integrity, abutment integrity, color match, secondary caries, gingival inflammation and abrasion of opposite teeth at different time points of recall. Only one case of onlay debonding in CAD/CAM IPS e.max CAD group, and one case of a small-scale collapse of functional cusp were recorded at the 12-month recall. Clinical performance of onlay restorations in the remaining was good. Conclusion Both CAD/CAM IPS e.max CAD and LavaTM Ultimate onlays can be used in repairing posterior pulpless teeth, showing an acceptable long-term clinical efficacy.
