掺铒钇铝石榴石激光在显微根尖手术治疗有窦型慢性根尖周脓肿中的疗效

摘　　要：目的观察应用掺铒钇铝石榴石激光(Er:YAGL)在显微根尖手术治疗有窦型慢性根尖周脓肿的疗效。方法选取2018年1月至2020年12月于合肥市口腔医院牙体牙髓就诊并诊断为有窦型慢性根尖周脓肿的患者90例。按照随机数字表法将患者分为试验组和对照组,各45例,试验组接受显微镜下Er:YAGL治疗,对照组则采用单纯显微镜下根尖手术治疗。锥体束CT(CBCT)比较术前和术后3个月两组患者根尖病变区域灰度值,术后6个月比较两组治疗成功率。结果试验组术前术后根尖区平均灰度值差值[(51.30±11.75)mg/cm^(3)]高于对照组[(37.91±13.60)mg/cm^(3)],差异有统计学意义(P=0.006)。术后6个月复查结果显示,试验组和对照组成功率分别为93.33%、86.67%,两组差异无统计学意义(P=0.380)。结论Er:YAGL在显微根尖手术治疗有窦型慢性根尖周脓肿中可有效改善骨密度,促进骨质修复,值得临床推广。Objective To observe the clinical effect of Er:YAG laser inapical microsurgeryin the treatment of chronic periapical abscess with sinus.Methods Ninety patients diagnosed with chronic periapical abscess were collected from the Department of Endodontics,Hefei Hospital of Stomatology from January 2018 to December 2020.According to the random number table,the patients were divided into the experimental group and thecontrol group(n=45).Patients in observation group were treated with Er:YAG laser under microscope,while control group with traditional apical surgery under microscope.Cone-beam CT(CBCT)was used to compare the gray value changes of apical lesions before and three months after surgery,and the clinical success rate was compared six months after surgery.Results The difference of average gray value of apical lesion area before and after operation[(51.30±11.75)mg/cm^(3)]was higher inexperimental groupthan that in control group[(37.91±13.60)mg/cm^(3)],with statistical significance(P=0.006).Six months after surgery,the success rate of experimental group andcontrol group was 93.33%and 86.67%respectively.There was no significant difference in the surgery success rate of these two groups(P=0.380).Conclusions Er:YAGL can effectively promote the bone repair of periapical lesion in the treatment of chronic periapical abscess with sinusinapical microsurgery,which is worth clinical promotion.
