摘  要:目的评价血红素结合蛋白(HPX)在大鼠脑缺血再灌注损伤中的作用。方法健康雄性SD大鼠120只,7~8周龄,体重250~280 g,采用随机数字表法分为假手术组(S组,n=36)、脑缺血再灌注组(I/R组,n=36)、溶剂对照组(V组,n=24)和HPX组(n=24)。采用线栓法阻塞大脑中动脉制备脑缺血再灌注损伤模型,缺血120 min后恢复灌注。S组和I/R组分别于再灌注6、12和24 h时,处死4只大鼠,取同侧大脑皮质缺血半暗带,采用Western blot法测定HPX表达水平。于再灌注即刻,I/R组、V组和HPX组侧脑室分别注射0.9%生理盐水、0.1%NaN_(3)和1.86 mg/ml的HPX各10μl。每组取8只大鼠,分别于再灌注1~7 d时,行神经功能缺损评分。于再灌注1和7 d时,每组处死8只大鼠,取脑组织,测定梗死体积,计算梗死体积百分比。结果与S组比较,I/R组再灌注24 h时大脑皮质缺血半暗带HPX表达上调,I/R组、V组和HPX组再灌注1~7 d时神经功能缺损评分降低,再灌注1和7 d时脑梗死体积百分比升高(P<0.05);与I/R组比较,HPX组再灌注1~7 d时神经功能缺损评分升高,再灌注1和7 d时脑梗死体积百分比降低(P<0.05),V组和I/R组上述指标差异无统计学意义(P>0.05)。结论HPX表达上调是大鼠脑缺血再灌注损伤的内源性保护机制。Objective To evaluate the role of hemopexin(HPX)in cerebral ischemia-reperfusion(I/R)injury in rats.Methods One hundred and twenty healthy male Sprague-Dawley rats,aged 7-8 weeks,weighing 250-280 g,were divided into sham operation group(S group,n=36),cerebral I/R group(I/R group,n=36),vehicle group(V group,n=24),and HPX group(n=24).The model of cerebral I/R injury was established by 120 min middle cerebral artery occlusion followed by reperfusion in anesthetized rats.At 6,12 and 24 h of reperfusion,4 rats in S group and I/R group were sacrificed,and the ischemic penumbra of the ipsilateral cerebral cortex was obtained to detect the expression of HPX by Western blot.In I/R,V and HPX groups,0.9%normal saline 10μl,0.1%NaN_(3)10μl,and 1.86 mg/ml HPX 10μl were injected into the lateral ventricle,respectively,immediately after reperfusion.Eight rats in each group were selected,and neurological deficit was scored at 1-7 days of reperfusion.Eight rats in each group were sacrificed at 1 and 7 days of reperfusion,brains were removed,and brain tissues were obtained for measurement of infarct size,and the percentage of infarct size was calculated.Results Compared with S group,the expression of HPX in cerebral ischemic penumbra was significantly up-regulated at 24 h of reperfusion in I/R group,and the neurological deficit scores were significantly decreased at 1-7 days of reperfusion,and the percentage of cerebral infarct size was increased at 1 and 7 days of reperfusion in I/R,V and HPX groups(P<0.05).Compared with I/R group,the neurological deficit scores were significantly increased at 1-7 days of reperfusion,and the percentage of cerebral infarct size was decreased at 1 and 7 days of reperfusion in HPX group(P<0.05),and no significant change was found in the above indicators in V and I/R groups(P>0.05).Conclusion Up-regulation of HPX expression is the endogenous protective mechanism of cerebral I/R injury in rats.
