针刺疗法联合卡马西平治疗三叉神经痛临床观察

摘　　要：目的综合性评价针刺治疗联合卡马西平对三叉神经痛患者的临床效果。方法选取2019年1月—2020年1月于抚顺市中医院神经内科病房住院并接受治疗的三叉神经痛患者60例,入组病例均采用信封法随机分为对照组和试验组,各30例。对照组予以西药卡马西平片口服治疗,试验组在对照组基础上联合针刺治疗方案,观察2组患者治疗前后中医证候积分、视觉疼痛模拟量表(Visual analogue scale,VAS)评分及临床疗效变化情况。结果经临床治疗,试验组中医证候积分为(4.31±1.12)分,VAS评分为(2.23±0.41)分,总有效率93.33%(28/30)。对照组中医证候积分为(7.29±1.53)分,VAS评分为(4.49±0.58)分,总有效率80.00%(24/30),试验组均优于对照组,差异有统计学意义(P<0.05)。结论三叉神经痛患者在口服卡马西平片基础上联合针刺治疗方案可改善局部面神经修复速率,降低疼痛刺激程度,恢复患者生活质量,扶正祛邪、化瘀清火、通络除痹,中西结合疗法合用可以提高机体免疫力,临床效果显著,值得临床推广。Objective To evaluate the clinical effect of acupuncture combined with carbamazepine on trigeminal neuralgia.Methods From January 2019 to January 2020,60 patients with trigeminal neuralgia who were hospitalized and treated in the department of neurology of Fushun Hospital of Traditional Chinese Medicine were randomly divided into control group and experimental group by envelope method.30 cases in the control group were treated by oral administration of carbamazepine,and 30 cases in the experimental group were treated by acupuncture on the basis of the control group.The changes of traditional Chinese medicine(TCM)syndrome scores,visual analogue scale(VAS)scores and clinical efficacy before and after treatment in 2 groups were observed.Results After clinical treatment,the TCM syndrome score and VAS score of the experimental group were(4.31±1.12)points and(2.23±0.41)points,with a total effective rate of 93.33%(28/30).The TCM syndrome score of the control group was(7.29±1.53)points,the VAS score was(4.49±0.58)points,and the total effective rate was 80.00%(24/30).The experimental group was better than the control group,and the difference was statistically significant(P<0.05).Conclusion On the basis of oral carbamazepine tablets,acupuncture treatment can improve the local facial nerve repair rate,reduce the degree of pain stimulation,restore the quality of life of patients,strengthening the pathogenic factors,removing blood stasis and clearing fire,clearing collateral,combination therapy of traditional Chinese and western medicine can improve the body immunity,clinical effect is significant,and is worthy of clinical promotion.
