磷霉素钠联合头胞哌酮舒巴坦治疗广泛耐药鲍曼不动杆菌呼吸机相关肺炎的临床观察

摘　　要：目的观察磷霉素钠联合头胞哌酮舒巴坦治疗广泛耐药鲍曼不动杆菌呼吸机相关肺炎(ventilator associated pneumonia,VAP)的临床效果。方法选择2014年1月—2016年12月医院重症医学科VAP、经痰培养证实对常用抗菌药物全耐药的鲍曼不动杆菌感染患者,共62例。给予磷霉素钠4~12 g/次,每日2次,头胞哌酮舒巴坦3.0 g/次,每日2次。直至菌培养显示鲍曼不动杆菌被清除或其他原因停药。结果62例患者中,中途退出6例,死亡4例,完成52例,鲍曼不动杆菌清除48例,清除率92.3%。停药后1周内死亡3例,1个月内死亡7例。结论磷霉素钠联合头胞哌酮舒巴坦对导致VAP的广泛耐药鲍曼不动杆菌的清除率达到92.3%,明显高于现有的其他疗法。Objective To observe the clinical effect of fosfomycin sodium combined with cefoperazone sulbactam in the treatment of ventilator associated pneumonia(VAP)caused by extensively resistant Acinetobacter baumannii.Methods From January 2014 to December 2016,62 patients with acinetobacter baumannii infection who were confirmed by sputum culture to be resistant to common antibiotics were selected.Fosfomycin sodium was given 4 to 12 g each time,twice a day.Cefoperazone sulbactam was given 3.0 g each time,twice a day.Until the culture indicates that Acinetobacter baumannii has been removed or the drug has been discontinued for other reasons.Results Among 62 cases,6 cases dropped out,4 cases died,52 cases completed,48 cases were cleared of acinetobacter baumannii,the clearance rate was 92.3%.Three patients died within one week and seven within one month after drug withdrawal.The cause of death was not directly related to acinetobacter baumannii infection.Conclusion The clearance rate of fosfomycin sodium combined with cefoperazone sulbactam on extensively resistant acinetobacter baumannii causing VAP reached 92.3%,which was significantly higher than other existing therapies.
