摘 要:目的检测淋巴细胞活化基因3(LAG-3)和程序死亡受体1(PD-1)在非小细胞肺癌(NSCLC)组织中的表达及二者之间关系,并分析其与NSCLC临床病理特征及预后的关系。方法选取2011-06-01-2016-06-01在山东省医学科学院附属医院经手术治疗的70例NSCLC患者病理标本。运用免疫组织化学方法检测LAG-3和PD-1在NSCLC组织中的表达情况。结果LAG-3和PD-1主要定位于肿瘤浸润淋巴细胞(TILs)的细胞质和细胞膜上.阳性表达率分别为51.4%(36/70)和58.6%(41/70);LAG3阳性表达与TNM分期(χ^(2)=5.803,P=0.016)和淋巴结转移(χ^(2)=5.042,P=0.025)有关联,PD-1阳性表达与TNM分期(χ^(2)=5.156,P=0.023)、淋巴结转移(χ^(2)=7.693,P<0.01)以及吸烟状况(χ^(2)=8.729,P<0.01)有关联。Spearman等级相关性分析显示,LAG-3与PD-1表达呈正相关(r=0.343,P=0.004)。Kaplan-Meier单因素分析显示,患者的总生存(OS)与病理类型、分化程度、TNM分期、淋巴结转移、LAG-3表达和PD-1表达有关联,P<0.05;多变量Cox回归分析发现,分化程度(HR=0.293.95%CI为0.131-0.655.P=0.003)、TNM分期(HR=3.151,95%CI为1.617~6.143,P=0.001)和PD-1(HR=2.285,95%CI为1.179~4.430,P=0.014)是OS.不良预后的独立影响因素。结论LAG-3和PD-1与NSCLC临床病理特征关系密切,且与患者的预后有关,两者表达具有一定正相关,其中PD-1是预后的独立影响因素。Objective To investigate the expressions and correlation of LAG-3 and PD-1 in NSCLC and to analyze the rela-tionships with clinicopathologic features and survival prognosis of NSCLC.Methods We ollected 70 surgical resection tissue specimens of NSCLC patients who underwent surgery from June 1,2011 to June 1.2016 at Affiliated Hospital of Shandong Academy of Medical Sciences.We used immunohistochemical methods to detect and evaluate the expression of LAG3 and PD-1 in NSCLC tissues.Results LAG-3 and PD-1 in NSCLC tissues were mainly located in the cytoplasm and cell membrane of TILs.The positive rates of LAG-3 and PD-1 were51.4%(36/70)and 58.6%(41/70)nespectively.The positive expression of LAG-3 was associated with TNM stage(χ^(2)=5.803,P=0.016)and lymph node metastasis(χ^(2)=5.042.P=0.025).The positive expression of PD-1 was associated with TNM stage(χ^(2)=5.156.P=0.023),lymph node metastasis(χ^(2)=7.693.P<0.01).and smoking states(χ^(2)=8.729,P<0.01).Spearman rank correlation analysis showed that LAG-3 was correlated with PD-1 expression(r=0.343.P=0.004).Univariate analysis by Kaplan-Meier indicated that OS was related to pathological type.differentiation grade.TNM stage.lymph node metastasis.LAG 3 expression and PD-1 expression(P<0.05).Multivariate analysis showed that differentiation grade(HR=0.293.95%CI:0.131-0.655.P=0.003).TNM stage(HR=3.151.95%CI:1.617-6.143.P=0.001)and PD-1 expression(HR=2.285.95%CI:1.179-4.430,P=0.014)were the independent prognostic factors influencing Os.Conclusions LAG 3 and PD-1 are as-sociated with eclinicopathological features of NSCLC and prognosis of patients.There is a positive correlation between LAG-3 and PD-1.PD-1 is an independent prognostic factor.
