摘　　要：目的探讨原发性开角型青光眼(POAG)视神经损伤患者血浆内皮素-1(ET-1)水平与外周血巨噬细胞中核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)炎症复合体、血清炎症因子水平及眼动脉血流的关系。方法选择2018年2月至2020年2月焦作市人民医院收治的117例POAG视神经损伤患者为研究对象,依据视野的平均缺损(MD)分为轻度损伤组(MD<7 dB,43例)、中度损伤组(7≤MD≤12 dB,35例)和重度损伤组(MD>12 dB,39例),比较3组患者血浆ET-1水平、血清白细胞介素(IL)-2、IL-4、IL-6、IL-8、IL-12、IL-18、IL-1β水平、外周静脉血巨噬细胞中NLRP3、凋亡相关微粒蛋白(ASC)、半胱氨酸天冬氨酸蛋白酶1(caspase-1)水平及眼动脉收缩期最大流速(PSV)、舒张末期血流速度(EDV)、阻力指数(RI),并分析血浆ET-1水平与外周血巨噬细胞中NLRP3炎症复合体、血清炎症因子水平及眼动脉血流的关系。结果3组患者血清IL-2、IL-4、IL-6水平比较差异无统计学意义(P>0.05);重度损伤组和中度损伤组患者血浆ET-1及血清IL-18、IL-1β水平显著高于轻度损伤组(P<0.05),重度损伤组患者血浆ET-1及血清IL-18、IL-1β水平显著高于中度损伤组(P<0.05);重度损伤组和中度损伤组患者血清IL-12水平显著低于轻度损伤组(P<0.05),重度损伤组患者血清IL-12水平显著低于中度损伤组(P<0.05)。重度损伤组和中度损伤组患者外周血巨噬细胞中NLRP3、ASC、caspase-1水平显著高于轻度损伤组(P<0.05),重度损伤组患者外周血巨噬细胞中NLRP3、ASC、caspase-1水平显著高于中度损伤组(P<0.05)。重度损伤组和中度损伤组患者眼动脉PSV、RI显著高于轻度损伤组,眼动脉EDV显著低于轻度损伤组(P<0.05);重度损伤组患者眼动脉PSV、RI显著高于中度损伤组,眼动脉EDV显著低于中度损伤组(P<0.05)。Pearson相关性分析显示,POAG视神经损伤患者血浆ET-1水平与血清IL-18、IL-1β及外周血巨噬细胞中NLRP3、ASC、Objective To investigate the relationship between plasma endothelin-1(ET-1)level and nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)inflammatory complex in peripheral blood macrophages,serum inflammatory factors and ophthalmic artery blood flow in patients with primary open angle glaucoma(POAG)and optic nerve injury.Methods A total of 117 POAG patients with optic nerve injury admitted to Jiaozuo People′s Hospital from February 2018 to February 2020 were selected as the research subjects.According to the mean defect(MD)of visual field,they were divided into mild injury group(MD<7 dB,43 cases),moderate injury group(7≤MD≤12 dB,35 cases)and severe injury group(MD>12 dB,39 cases).The levels of ET-1 in plasma and interleukin(IL)-2,IL-4,IL-6,IL-8,IL-12,IL-18,IL-1 in serum and NLRP3,apoptosis-associated speck like protein containing CARD(ASC),caspase-1 in peripheral venous blood macrophages,and the peak systolic velocity(PSV),end diastolic velocity(EDV),resistance index(RI)of ophthalmic artery were compared among the three groups.The relationship between plasma ET-1 level and NLRP3 inflammatory complex in peripheral blood macrophages,serum inflammatory factors and ophthalmic artery blood flow was analyzed.Results There was no significant difference in the levels of serum IL-2,IL-4 and IL-6 among the three groups(P>0.05).The levels of plasma ET-1 and serum IL-18,IL-1βin the severe injury group and moderate injury group were significantly higher than those in the mild injury group(P<0.05).The levels of plasma ET-1 and serum IL-18,IL-1βin the severe injury group were significantly higher than those in the moderate injury group(P<0.05).The level of serum IL-12 in the severe injury group and moderate injury group was significantly lower than that in the mild injury group(P<0.05),and the level of serum IL-12 in the severe injury group was significantly lower than that in the moderate injury group(P<0.05).The levels of NLRP3,ASC and caspase-1 in peripheral blood macrophages in the severe injury
