海马神经可塑与脑卒中后情志病症关系的研究进展

摘　　要：脑卒中后可发生血管性痴呆(VD)、脑卒中后抑郁(PSD)等情志病症,且与海马神经可塑密切相关。海马是神经重要核团之一,参与人体多种生理病理过程,对人体情绪变化也有重要影响。而脑卒中后伴发的情志病症(如PSD和VD)存在海马神经可塑障碍,主要表现为神经细胞萎缩、活性降低以及神经突触改变等。神经递质、神经营养因子及信号通路系统等均可能参与海马神经可塑,进而影响PSD的发病。脑卒中后存在的海马神经可塑为PSD及VD的早期预防提供了可能,对脑卒中进行早期干预并调节其神经可塑有望阻断脑卒中向PSD和VD进展。目前国内关于脑卒中后PSD和VD的研究仍较少,脑卒中后情志病症的演变及病因病机还有待进一步探讨和研究。Emotional disorders such as vascular dementia(VD)and post-stroke depression(PSD)can occur after stroke,and are closely related to hippocampal nerve plasticity.The hippocampus is one of the important nerve nuclei,which participates in a variety of physiological and pathological processes in the human body,and also has an important influence on the emotional changes of the human body.However,the emotional disorders(such as PSD and VD)associated with stroke have plastic disorders in the hippocampus,which are mainly manifested as atrophy of nerve cells,decreased activity,and changes in nerve synapses.Neurotransmitters,neurotrophic factors,and signaling pathway systems may all participate in hippocampal nerve plasticity,which in turn affects the pathogenesis of PSD.The hippocampal nerve plasticity that exists after stroke provides the possibility for the early prevention of PSD and VD.Early intervention and adjustment of the neuroplasticity of the patients are expected to block the progress of stroke to PSD main VD.At present,domestic research on this aspect is relatively rare,and the evolution,etiology and pathogenesis of emotional disorders after stroke need to be further explored and studied.
