翼状胬肉切除联合游离结膜瓣移植术及治疗性角膜接触镜治疗翼状胬肉疗效的评估

摘　　要：目的探讨翼状胬肉切除联合游离结膜瓣移植术及治疗性角膜接触镜治疗翼状胬肉的疗效。方法将25例(30只眼)原发翼状胬肉患者随机分为A、B两组。A组12例(15只眼)行翼状胬肉切除联合游离结膜瓣移植术及术后佩戴治疗性角膜接触镜进行治疗;B组13例(15只眼)行单纯翼状胬肉切除术。随访6~12个月。结果A组术后结膜瓣移植片平整,2~3(2.3±1.1)d疼痛基本消失。B组患者术后基本有有异物疼痛感,5~7(5.6±2.3)d疼痛基本消失。两组比较,差异有统计学意义(P<0.05)。A组术后,角膜上皮4~6(4.2±1.7)d基本修复。B组术后,角膜上皮7~9(7.3±3.2)d基本修复。两组比较,差异有统计学意义(P<0.05)。A组15只眼中有1只眼复发,复发率为6%;B组15只眼中有4只眼复发,复发率为26%。两组比较,差异有统计学意义(P<0.05)。结论翼状胬肉切除联合游离结膜瓣移植术及治疗性角膜接触镜治疗翼状胬肉的效果较好,可降低胬肉复发率。ObjectiveTo investigate the efficacy of pterygium excision combined with free conjunctival flap transplantation and therapeutic corneal contact lens in the treatment of pterygium.Methods25 patients(30 eyes)with primary pterygium were randomly divided into group A and group B.In group A,12 cases(15 eyes)underwent pterygium excision combined with free conjunctival flap transplantation and postoperative wearing therapeutic corneal contact lens,and 13 cases(15 eyes)in group B underwent simple pterygium excision.The patients were followed up for 6 to 12 months.ResultsThe conjunctival flap grafts were smooth in group A,and the pain disappeared in 2 to 3 days(mean 2.3±1.1 days).The patients in group B generally had foreign body pain after operation,and the pain basically disappeared in 5 to 7 days(mean 5.6±2.3 days).There was significant difference between the two groups(P<0.05).In group A,the corneal epithelium was repaired for 4 to 6 days(mean 4.2±1.7 days).In group B,corneal epithelium was repaired for 7 to 9 days(mean 7.3±3.2 days).There was significant difference between the two groups(P<0.05).In group A,1 patient had recurrence in 15 eyes,with a recurrence rate of 6%.In group B,4 of 15 patients had recurrence,with a recurrence rate of 26%.There was significant difference between the two groups(P<0.05).ConclusionsPterygium excision combined with free conjunctival flap transplantation and therapeutic corneal contact lens are effective in the treatment of pterygium and can reduce the recurrence rate of pterygium.
