自体骨植入联合富血小板纤维蛋白在充填自体牙移植牙周骨缺损中的效果

摘　　要：目的探讨自体牙即刻移植中,自体骨植入联合富血小板纤维蛋白(PRF)在充填牙周骨缺损中的应用效果。方法将自体牙移植患者55例按牙周骨缺损区填充物不同分为2组:对照组25例给予PRF填充牙周骨缺损,观察组30例给予自体骨植入联合PRF填充牙周骨缺损。比较2组完全愈合率、治疗成功率及总有效率、局部感染率。结果与对照组比较,观察组完全愈合率、治疗成功率及总有效率均显著升高(93.33%比68.00%、83.33%比52.00%、93.33%比76.00%,χ^(2)=4.303、6.269、3.296,P=0.038、0.023、0.049);2组局部感染率比较差异无统计学意义(83.33%比80.00%,χ^(2)=0.001、P=0.974)。结论自体牙即刻移植中,自体骨植入联合富血小板纤维蛋白在充填牙周骨缺损的愈合率更高,疗效更好。Objective To investigate the efficacy of autogenous bone implantation combined with platelet-rich fibrin(PRF)in filling periodontal bone defect in immediate autogenous tooth transplantation.Methods Fifty-five patients undergoing autogenous tooth transplantation were given PRF alone(control group,25 patients)or in combination with autogenous bone implantation(observation group,30 patients)for filling periodontal bone defect.The complete healing rate,treatment success rate,total effective rate and local infection rate were compared between the two groups.Results Compared with the control group,the combination of autogenous bone implantation and PRF significantly increased the complete healing rate(93.33%vs 68.00%,χ^(2)=4.303,P=0.038),treatment success rate(83.33%vs 52.00%,χ^(2)=6.269,P=0.023),and total effective rate(93.33%vs 76.00%,χ^(2)=3.296,P=0.049).There was no significant difference in the local infection rate between the two groups(83.33%vs 80.00%,χ^(2)=0.001,P=0.974).Conclusion In immediate autogenous tooth transplantation,autogenous bone implantation combined with PRF is associated with a higher healing rate and a better curative effect in filling periodontal bone defect.
