摘　　要：[目的]定量检测门静脉血microRNA-497(miR-497)表达量,探讨其与结直肠癌(colorectal cancer,CRC)肝转移及预后的关系。[方法]应用实时荧光定量RT-PCR检测177例行根治术CRC患者术中门静脉和术前外周血miR-497表达量,术后随访3年,选择30位健康志愿者空腹静脉血作对照组,分析术中门静脉血miR-497表达量与CRC肝转移及预后的关系。[结果]与正常对照组空腹静脉血比较,CRC组术中门静脉血及术前外周血miR-497表达量均显著降低(P<0.01)。CRC组中同时性肝转移者、异时性肝转移者术中门静脉血及术前外周血miR-497表达量均分别显著低于无同时性肝转移者、无异时性肝转移者(P<0.01),异时性肝转移者术中门静脉血miR-497表达量显著低于其术前外周血表达量(P<0.05)。CRC组术中门静脉血miR-497表达量与肿瘤分化程度、TNM分期、淋巴结转移均呈负相关(P<0.01),与术后生存期长短呈正相关(P<0.01);单因素分析显示术中门静脉血及术前外周血miR-497表达量均与CRC异时性肝转移相关,Cox多因素分析显示两者是其独立危险因素(EXP=9.782,4.285)。[结论]术中门静脉血miR-497表达量变化与CRC重要病理特征及其肝转移、预后密切相关,对CRC异时性肝转移的预测敏感性高于外周血miR-497,可成为CRC术后的预后评价指标。[Objective]To investigate the correlation between the level of miR-497in portal vein blood and liver metastasis and prognosis of colorectal cancer(CRC).[Methods]The expression of miR-497in the portal vein blood abstained during operation and in the peripheral blood before the operation were detected by real-time fluorescence quantitative RT-PCR in 177patients with CRC.The postoperative 3years prognosis was followed-up.The relationship between the intraoperative portal vein blood miR-497expression levels and the liver metastasis and prognosis of CRC were analyzed.[Results]The miR-497expression levels in intraoperative portal vein blood and preoperative peripheral blood in the CRC synchronous liver metastases group and the metachronous liver metastases group were respectively significantly lower than those in the no synchronous liver metastases group and the no metachronous liver metastases group(P<0.01).The miR-497expression levels in intraoperative portal vein blood were significantly lower than those in preoperative peripheral blood in metachronous liver metastases group(P<0.05).The miR-497expression levels in intraoperative portal vein blood of CRC group were negatively related with the differentiated degree of tumor,TNM stage,lymph node metastasis(P<0.01),and were positively related with the postoperative survival(P<0.01).Univariate analysis revealed that the miR-497expression levels in intraoperative portal vein blood and preoperative peripheral blood were all correlated to metachronous liver metastases from CRC.Multivariate Cox analysis showed that they were independent risk factors for metachronous liver metastases from CRC(EXP=9.782,4.285).[Conclusion]The miR-497expression levels in intraoperative portal vein blood are relative with the important pathologic feature,prognosis and liver metastasis of CRC and its predictive sensitivity of CRC metachronous liver metastases is higher than miR-497in preoperative peripheral blood,and can be a prognostic marker of CRC.
