内侧半月板损伤手术前后的三维步态分析

摘　　要：[目的]探讨内侧半月板损伤患者在手术治疗前后的三维步态变化情况。[方法]研究对象分为两组,损伤组为本院关节外科2019年5月—2020年5月关节镜手术证实内侧半月板损伤的17例患者;健康组为同期招募的健康体检正常人17例。行三维步态特征分析,比较损伤组术前、损伤组术后,以及健康人三维步态数据。[结果]损伤组术前步长及步速显著低于健康组(P<0.05)。损伤组术前内收角度、内收外展关节活动度(range of motion,ROM)、摆动相最大弯曲角度、支撑相最大伸展角度、承重期最大弯曲角度以及屈曲伸展ROM较健康组显著降低(P<0.05)。损伤组术后内收角度、内收外展ROM、支撑相最大伸展角度、承重期最大弯曲角度、屈曲伸展ROM较术前显著改善,但仍未达到正常范围(P<0.05)。损伤组各动力学参数均较健康组显著降低(P<0.05),损伤组术后内收力矩第二峰值较术前显著提高(P<0.05)。[结论]内侧半月板损伤患者行关节镜手术后,虽然三维步态参数较术前有一定恢复,但是仍未能完全达到正常水平范围内,故术后仍需加强康复训练。[Objective]To explore the three-dimensional gait changes of patients with medial meniscus injury before and after surgery.[Methods]The research subjects were divided into two groups,the patients who underwent arthroscopic surgery for confirmed diagnosis of medial meniscus injury in our hospital from May 2019 to May 2020 were fall into the injury group,while 17 healthy persons who received routine healthy examination during the same period were recruited as the healthy group.The three-dimensional gait assay was conducted on the subjects,including the patients in the injury group preoperatively and at the latest follow-up and the healthy persons in the healthy group.The three-dimensional gait data were compared among the patients preoperatively and postoperatively,and the healthy persons.[Results]The injury group preoperatively had significantly less step length and pace than the healthy group(P<0.05).The injury group preoperatively had significantly less adduction angle,adduction and abduction ROM,swing phase maximum bending angle,and support phase maximum extension angle,maximum bending angle during load-bearing period,and flexion extension ROM than healthy group(P<0.05).Although the injury group postoperatively got significantly improvements in adduction angle,adduction and abduction ROM,maximum extension angle of support phase,maximum bending angle during load-bearing period,flexion extension in injury group ROM compared with those preoperatively(P<0.05),all abovesaid parameters postoperatively still did not reach the normal range of the healthy group(P<0.05).In term of dynamic parameters,injury group proved significantly inferior to the healthy group(P<0.05),despite of the fact that the injury group got a significant increase of second peak of adduction torque postoperatively compared with that preoperatively(P<0.05).[Conclusion]For medial meniscus injury,there are improvements in some extent in term of three-dimensional gait analysis after arthroscopic surgery,while which have not fully reached the normal ran
