MCV、血红蛋白电泳及红细胞脆性试验联合在地中海贫血筛查中的应用价值

摘　　要：目的探讨MCV、血红蛋白电泳及红细胞脆性试验联合检测在地中海贫血筛查中的应用价值。方法方便选择该院2018年1月—2019年6月收治的82例地中海贫血患者作为研究组,选择同期非地中海患者102例作为对照组,比较结果。结果两组3项指标检测结果,差异有统计学意义(P<0.05)。MCV筛查特异度89.22%低于血红蛋白电泳,差异有统计学意义(χ^(2)=4.964,P<0.05),而血红蛋白电泳灵敏度87.80%及红细胞脆性试验灵敏度91.46%低于MCV灵敏度98.78%,差异有统计学意义(P<0.05)。平行检测灵敏度100.00%高于系列检测,差异有统计学意义(χ^(2)=6.856,P<0.05),平行检测特异度82.35%低于系列检测100.00%,差异有统计学意义(χ^(2)=6.925,P<0.05)。结论MCV、血红蛋白电泳及红细胞脆性试验联合检测对地中海贫血的筛查价值显著。Objective To explore the application value of combined detection of MCV,hemoglobin electrophoresis and red blood cell fragility test in thalassemia screening.Methods The 82 thalassemia patients admitted to the hospital from January 2018 to June 2019 were convenient selected as the study group,and 102 non-Mediterranean patients during the same period were selected as the control group,and the results were compared.Results There were statistically significant differences in the test results of the three indicators between the two groups(P<0.05).The specificity of MCV screening was 89.22%,which was lower than that of hemoglobin electrophoresis,the difference was statistically significant(χ^(2)=4.964,P<0.05),while the sensitivity of hemoglobin electrophoresis was 87.80%and the sensitivity of red blood cell fragility test was 91.46%,which was lower than that of MCV 98.78%,the difference was statistically significant(P<0.05).The sensitivity of parallel detection of 100.00%was higher than that of serial detection,the difference was statistically significant(χ^(2)=6.856,P<0.05),and the specificity of parallel detection of 82.35%was lower than that of serial detection 100.00%,the difference was statistically significant(χ^(2)=6.925,P<0.05).Conclusion The combined detection of MCV,hemoglobin electrophoresis and red blood cell fragility test has significant value in screening for thalassemia.
