长脉冲1064 nm Nd:YAG激光联合DPL和外用马来酸噻吗洛尔滴眼液治疗婴儿血管瘤的疗效分析

摘　　要：目的分析长脉冲1064 nm Nd:YAG激光联合窄谱强脉冲光(DPL)和外用马来酸噻吗洛尔滴眼液治疗婴儿血管瘤的临床疗效。方法将江门市妇幼保健院皮肤科门诊2018年5月至2020年3月收治的104例婴儿血管瘤患儿随机分成4组,每组26例。药物组:采用外用马来酸噻吗洛尔滴眼液治疗;激光组:采用外用长脉冲1064 nm Nd:YAG激光治疗;联合1组:采用长脉冲1064 nm Nd:YAG激光联合外用马来酸噻吗洛尔滴眼液治疗;联合2组:采用长脉冲1064 nm Nd:YAG激光联合DPL和外用马来酸噻吗洛尔滴眼液治疗。对比分析4组的临床疗效。结果治疗6个月后,联合2组总有效率高于联合1组、激光组、药物组,组间差异有统计学意义(P<0.05)。联合2组治疗6个月后痊愈患儿激光次数少于联合1组和激光组,组间差异有统计学意义(P<0.05)。联合2组不良反应发生率低于联合1组、激光组、药物组,组间差异有统计学意义(P<0.05)。结论血管瘤患儿采用长脉冲1064 nm Nd:YAG激光联合DPL和马来酸噻吗洛尔滴眼液治疗,能够提高疗效,减少激光治疗次数,缩短病程,减少不良反应。Objective To analyze the clinical efficacy of long-pulse 1064 nm Nd:YAG laser combined with narrow-spectrum intense pulsed light(DPL)and topical timolol maleate eye drops in the treatment of infantile hemangioma.Methods A total of 104 children with infantile hemangioma admitted to the Dermatology Clinic of Jiangmen Maternal and Child Health Hospital from May 2018 to March 2020 were randomly divided into four groups,each with 26 cases.Drug group:treated with topical timolol maleate eye drops;laser group:treated with topical long pulse 1064 nm Nd:YAG laser;combination group 1:treated with long pulse 1064 nm Nd:YAG laser combined with topical timolol maleate eye drops treatment;combined group 2:long-pulse 1064 nm Nd:YAG laser combined with DPL and topical timolol maleate eye drops treatment.Comparative analysis of the clinical efficacy of the four groups.Results After 6 months of treatment,the total effective rate of the combination group 2 was higher than that of the combination group 1,laser group and drug group,and the difference between the groups was statistically significant(P<0.05).The number of lasers in the children who recovered after 6 months of treatment in the combined group 2 was less than that in the combined group 1 and laser group,and the difference between the groups was statistically significant(P<0.05).The incidence of adverse reactions in the combination group 2 was lower than that in the combination group 1,laser group and drug group,and the difference between the groups was statistically significant(P<0.05).Conclusions Children with hemangioma are treated with long-pulse 1064 nm Nd:YAG laser combined with DPL and timolol maleate eye drops,which can improve the efficacy,reduce the number of laser treatments,shorten the course of the disease,and reduce adverse reactions.
