无抽搐电休克联合喹硫平治疗中老年双相情感障碍躁狂发作42例疗效观察

摘　　要：目的:探讨无抽搐电休克联合喹硫平治疗中老年双相情感障碍躁狂发作患者的疗效及认知功能的影响。方法:选取2018年6月至2019年6月江西省精神病院收治的中老年双相情感障碍躁狂发作患者84例作为研究对象,随机数字表法分为对照组(n=42)和观察组(n=42)。对照组采用无抽搐电休克治疗,观察组在对照组基础上加用喹硫平治疗。对两组临床疗效、Bech-Rafaelsen躁狂量表(BRMS)评分、认知功能及不良反应发生情况进行分析和比较。结果:观察组总有效率(92.86%)高于对照组(76.19%),差异有统计学意义(P<0.05)。治疗后观察组BRMS评分低于对照组,差异有统计学意义(P<0.05)。治疗后观察组持续操作测验(CPT)高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生情况差异无统计学意义(P>0.05)。结论:无抽搐电休克联合富马酸喹硫平片治疗双相情感障碍躁狂发作的效果显著,能明显改善患者病情严重程度,提升患者的注意力,安全有效。Objective:To investigate the effect of non-convulsive electric shock combined with quetiapine in the treatment of middle-aged and elderly patients with bipolar disorder and the effect of cognitive function.Methods:84 middle-aged and elderly patients with bipolar disorder manic episode admitted to Jiangxi Provincial Mental Hospital from June 2018 to June 2019 were selected as the research objects and randomly divided into control group(n=42)and observation group(n=42).The control group was treated with non-convulsive electroconvulsive therapy,and the observation group was treated with quetiapine on the basis of the control group.The clinical efficacy,Bech rafaelsen mania scale(BRMS)score,cognitive function and adverse reactions of the two groups were analyzed and compared.Results:The total effective rate of the observation group(92.86%)was higher than that of the control group(76.19%),the difference was statistically significant(P<0.05).After treatment,the BRMS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the CPT of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The effect of non-convulsive electric shock combined with quetiapine fumarate tablets in the treatment of manic episodes of bipolar disorder is significant,which can significantly improve the severity of the patient's condition and prompt the patient's attention,safety and effectiveness.
