摘  要:目的探讨骨贝类制剂对豚鼠消化性溃疡的疗效及对胃黏膜表皮生长因子(EGF)及表皮生长因子受体(EGFR)的影响,阐明骨贝类制剂对消化性溃疡的可能作用机制。方法2019年1月选取豚鼠80只,取20只作为对照组,其余60只采用水杨酸和冷热及惊吓等不良情绪刺激法建立胃溃疡豚鼠模型,并采用随机数字法分为安慰剂组、抑酸组和骨贝类组,每组20只。观察各组豚鼠摄食量、活动量和精神状况。测量溃疡最大横径和纵径、黏膜肌层缺损宽度,计算溃疡指数和溃疡面积。伊红-苏木素染色观察各组豚鼠胃溃疡病理形态学变化。免疫组织化学染色测定各组豚鼠胃黏膜EGF、EGFR表达情况。结果对照组豚鼠用药结束时摄食量、活动量和精神状态正常;安慰剂组豚鼠摄食量和活动量明显减少,精神状态差;抑酸组、骨贝类组豚鼠摄食量、活动量和精神状态基本正常。与安慰剂组比较,抑酸组、骨贝类组豚鼠胃溃疡指数、溃疡面积、黏膜肌层缺损宽度均明显降低,差异均有统计学意义(P<0.05);抑酸组豚鼠胃溃疡指数、溃疡面积、黏膜肌层缺损宽度与骨贝类组比较,差异均无统计学意义(P>0.05)。与安慰剂组比较,抑酸组、骨贝类组豚鼠溃疡旁胃黏膜EGF、EGFR光密度值均明显降低,骨贝类组豚鼠溃疡旁胃黏膜EGF、EGFR光密度值明显高于抑酸组,差异均有统计学意义(P<0.05)。对照组豚鼠胃黏膜形态结构正常;安慰剂组豚鼠胃黏膜有大量坏死组织,溃疡底部有瘢痕和肉芽组织形成,大量炎症细胞浸润;抑酸组、骨贝类组豚鼠溃疡底部有肉芽组织形成,溃疡浅,炎症细胞浸润及溃疡程度均较轻。结论骨贝类制剂对豚鼠消化性溃疡的疗效与奥美拉唑相当,在提高胃黏膜EGF、EGFR表达方面优于奥美拉唑。Objective To investigate the effect of bone shellfish on guinea pig peptic ulcer and its effect on epidermal growth factor(EGF)and epidermal growth factor receptor(EGFR)in gastric mucosa in order to elucidate the possible mechanism of bone shellfish on peptic ulcer.Methods 80 guinea pigs were selected in January 2019,20 guinea pigs were taken as control group,the remained 60 guinea pigs were established gastric ulcer guinea pig models by using salicylic acid and adverse emotional stimulation such as cold,heat and shock,and then were divided into placebo group,acid suppression group and bone shellfish group by random number method,with 20 rats in each group.The food intake,activity and mental status were observed.The maximum transverse diameter and longitudinal diameter of the ulcer and the width of the mucosal muscle defect were measured.The ulcer index and ulcer area were calculated.The pathological changes of gastric ulcer were observed by Hematoxylin-Eosin(HE)staining.Immunohistochemical staining was used to determine the expression of EGF and EGFR in gastric mucosa.Results The food intake,activity and mental state of guinea pigs were normal in the control group at the end of the treatment;The food intake and activity had significantly reduced,and had a poor mental state in the placebo group;The food intake,activity and mental state were basically normal in the acid suppression group and bone shellfish group.Compared with the placebo group,the gastric ulcer index,ulcer area and mucosal muscle defect width of the guinea pigs in the acid suppression group and the bone shellfish group were all decreased with statistical significance(P<0.05).There were no significant difference in gastric ulcer index,ulcer area and mucosal muscle defect width in guinea pigs between the acid suppression group and bone shellfish group(P>0.05).Compared with the placebo group,the EGF and EGFR optical density values of gastric mucosa in the acid suppression group and bone shellfish group were decreased.The EGF and EGFR optical density
