宫颈管分泌物Th1/Th2亚群与高危型HPV感染相关性研究

摘　　要：目的探讨宫颈管分泌物Th1/Th2亚群与高危型HPV感染的相关性。方法选取2018年3月至2019年3月在浙江省中医院行宫颈管分泌物检测提示高危型HPV阳性的60例患者为观察组,阴性的60例受检者为对照组,年龄均为18~35岁。两组受检者均留取宫颈管分泌物,采用人Th1/Th2亚群检测试剂盒(流式荧光法)进行细胞因子检测,选取Th1家族中的IL-2及Th2家族中的IL-10作为观测指标。分析比较两组结果,评估宫颈管分泌物Th1/Th2亚群与高危型HPV感染的相关性。结果观察组患者宫颈管分泌物IL-2、IL-10、IL-2/IL-10水平分别为(21.693±4.985)pg/ml、(30.448±5.496)pg/ml、(0.737±0.231),对照组分别为(32.868±5.328)pg/ml、(24.729±4.251)pg/ml、(1.366±0.310)。与对照组相比,观察组患者宫颈管分泌物IL-2、IL-2/IL-10水平均降低,IL-10水平升高,差异均有统计学意义(均P<0.05)。观察组患者宫颈病变越严重,宫颈管IL-2水平越低(r=-0.7238,P<0.05),IL-10水平越高(r=0.7657,P<0.05),IL-2/IL-10水平越低(r=-0.9277,P<0.05)。结论高危型HPV感染患者宫颈管分泌物Th1/Th2亚群失衡,Th1向Th2漂移,且宫颈病变严重程度与Th1/Th2亚群失衡相关。Objective To investigate the correlation between Th1/Th2 subsets in cervical canal and high-risk HPV infection.Methods From March 2018 to March 2019,60 patients aged 18-35 years with positive high-risk HPV infection(study group)were enrolled in the study,and 60 patients aged 18-35 years with negative high-risk HPV infection served as the control group.The secretion of cervical canal was collected from both groups,and IL-2 and IL-10 levels were detected by flow cytometry.The correlation between Th1/Th2 subsets and high-risk HPV infection was evaluated.Results The levels of IL-2,IL-10 and IL-2/IL-10 in cervical secretion of study group were(21.693±4.985)pg/ml,(30.448±5.496)pg/ml and(0.737±0.231).Those in the control group they were(32.868±5.328)pg/ml,(24.729±4.251)pg/ml and(1.366±0.310),respectively(P<0.05).The severity of the cervical lesions was positively correlated with the IL-10 level(r=0.7657,P<0.05),and negatively correlated with the IL-2 level(r=-0.7238,P<0.05)and IL-2/IL-10(r=-0.9277,P<0.05).Conclusion In patients with high-risk HPV infection,there is an imbalance of Th1/Th2 subsets in secretion of cervical canal,and the severity of cervical lesions is related to the imbalance of Th1/Th2 subsets.
