摘　　要：目的分析人乳头瘤病毒(HPV)E6/E7 mRNA检测和液基薄层细胞学检查(TCT)在宫颈癌筛查中的应用价值。方法选择2018年9月-2020年9月在丽水市人民医院进行宫颈癌筛查的963例已婚女性,根据筛查方法不同分为TCT+HPV DNA组(525例)和TCT+HPV E6/E7 mRNA组(438例)。比较HPV E6/E7 mRNA检测和TCT在宫颈癌筛查中的应用价值。结果TCT+HPV E6/E7 mRNA组初筛阳性率为32.00%(140/438),低于TCT+HPV DNA组的39.00%(204/525)(P<0.05)。两组未见上皮内病变细胞(NILM)、低级别鳞状上皮内病变(LSIL)、高级别鳞状上皮内病变(HSIL)患者筛查阳性率比较差异均有统计学意义(均P<0.05)。两组阴道镜转诊率及满意度比较差异均有统计学意义(均P<0.05)。TCT+HPV E6/E7 mRNA组宫颈癌筛查的灵敏度、特异度及准确度分别为37.19%、98.47%及64.61%,均高于TCT+HPV DNA组的19.93%、67.52%及41.14%,差异均有统计学意义(均P<0.05)。结论HPV E6/E7 mRNA和TCT在宫颈癌筛查中的应用价值较高。Objective To analyze the application value of human papilloma virus(HPV)E6/E7 mRNA test and Thinprep cytology test(TCT)in cervical cancer screening.Methods From September 2018 to September 2020,963 married women undergoing cervical cancer screening in Lishui People’s Hospital were selected and divided into TCT+HPV DNA group(525 cases)and TCT+HPV E6/E7 mRNA group(438 cases)according to screening methods,the application value of HPV E6/E7 mRNA test and TCT in cervical cancer screening was compared.Results The positive rate of preliminary screening in TCT+HPV E6/E7 mRNA group was 32.00%(140/438),which was statistically significantly lower than that in TCT+HPV DNA group(39.00%,204/525)(P<0.05).There were statistically significant differences in screening positive rates in patients with negative for intraepithelial lesions or malignancy(NILM),low-grade squamous intraepithelial lesion(LSIL),high-grade squamous intraepithelial lesion(HSIL)between the two groups(P<0.05).There were statistically significant differences in referral rate of colposcopy and satisfaction degree between the two groups(P<0.05).The sensitivity,specificity,and accuracy of cervical cancer screening in TCT+HPV E6/E7 mRNA group were 37.19%,98.47%,and 64.61%,respectively,which were statistically significantly higher than those in TCT+HPV DNA group(19.93%,67.52%,41.14%)(P<0.05).Conclusion The application value of HPV E6/E7 mRNA test and TCT in cervical cancer screening is high.
