摘 要:目的探讨多普勒超声在乳腺癌高危人群中的筛检价值。方法选取2015年6月至2020年6月间陕西省榆林市第一医院检测的105例乳腺癌高危人群,均行多普勒超声检查。以病理诊断为金标准,将患者分为良性组75例和恶性组30例,分析多普勒超声在乳腺癌高危人群中的筛检价值。结果病理诊断良性患者75例,恶性患者30例,多普特超声检测恶性患者27例,良性患者78例,两者比较,差异无统计学意义(P>0.05)。两组患者超声图像特征中,病灶形态、边界、纵横比、后方回声、血流分级和微钙化比较,差异均有统计学意义(均P<0.05);两组患者病灶生长方位和内部回声比较,差异无统计学意义(P>0.05)。两组患者超声造影特征中,渡越时间和始增时间比较,差异无统计学意义(P>0.05);两组患者上升支斜率、峰值强度和达峰时间比较,差异均有统计学意义(均P<0.05)。以病理诊断为金标准,超声诊断为乳腺癌者27例,准确率为90%,误诊1例,漏诊2例,敏感性为85%,特异性为89%。结论多普勒超声在乳腺癌高危人群的筛检中,具有无创、准确率较高、无不良反应等优点,值得临床应用推广。Objective To investigate the screening value of Doppler ultrasound for breast cancer in high-risk population. Methods One hundred and five high-risk individuals who toke physical examination for breast cancer at Yulin First Hospital were selected from June 2015 to June 2020. Doppler ultrasound examination was performed on them. Pathological diagnosis results were considered as gold standard,patients were categorized into a benign group( 75 patients) and a malignant group( 30 patients). The screening value of Doppler ultrasound for breast cancer was analyzed in high-risk population. Results There were 75 patients with benign disease and 30 patients with malignant disease confirmed by pathological diagnosis,and27 patients with malignant disease and 78 patients with benign disease confirmed by doppler ultrasound( P>0.05). By comparing the ultrasonic image features between the two groups,it can be found that there are significant differences in the image features of lesion morphology,boundary,aspect ratio,posterior echo,blood flow grading and microcalcification between the two groups( all P<0.05). There was no significant difference in growth orientation and internal echo between the two groups( P>0.05). By comparing the characteristics of image features between the two groups,it was found that there was no significant difference in transit time and initial increment time between the two groups( P>0.05),and there were significant differences in slope of ascending branches,peak intensity and peak arrival time between the two groups( P<0.05). With pathological diagnosis as the gold standard,27 cases of breast cancer were diagnosed by ultrasound,with an accuracy rate of 90%;1 patient was misdiagnosed,with a misdiagnosis rate of3. 3%;2 patients were missed diagnosis,with a missed diagnosis rate of 6. 7%,a sensitivity of 85% and a specificity of 89%. Conclusion Doppler ultrasound is non-invasive,high accurate for breast cancer screening in high-risk population without causing adverse reactions. It is suitable for all pati
